Work Methodology (Explain the step by step sequence of how the work will be done) Environmental Controls
1. Jacking nuts adjusted to pre-determined RL levels Standard Working Hours are:
2. • Monday – Friday: 7:00am – 6:00pm
Paint base of the posts
• Saturday 8:00 am – 1:00 pm
3. A forklift with jib attachment will sling and lift the safety screen posts Water discharge requires a Permit to Pump
4. Once lifted the post will be placed on top of Jacking nuts and double check the squareness with the barrier No entry to ‘No-Go’ areas without approval from the environment
5. team prior to works
After placing the posts the nuts to be tightened to secure the base plate before removing slings
Erosion and Sediment controls to be installed as per the Erosion and
6. Grouted before mesh installed – Grouting works to be completed by RCPBJV Sediment Control Plan. If conditions or activities change, advise
7. Environment Team to approve a revised Erosion and Sediment
Then the forklift with jib attachment will lift the safety mesh and install them ( will be placed against the posts) Control Plan. Notify foreman immediately of any damaged or
8. moved controls, or if controls at capacity are observed.
Securing the safety mesh on the post
Ensure all waste, including cigarette butts, are appropriately
9. EWP will be used for fastening bolts to top of safety screen mesh/cover plates and to unhook the lifting gear disposed. Recyclable materials should be separated.
10. The lifting gear will be unhooked once the mesh is secured on the posts Fuels and chemical to be bunded to 110% of the total volume
11. Securing the mobile plant after use Spill kits to be stocked and available at all times.
12. Refuelling the mobile plant Ensure all stockpiles are signposted with a stockpile number, and
stockpiles of asbestos containing material are covered and
13. Conduct housekeeping signposted with ‘Asbestos’ signage.
14. Client to check and sign off Ensure any material tracked onto the road is removed with street
15. sweepers.
Dust to be managed using watercarts and driving slowly over
16. REMOVING THE POSTS:
exposed areas when required.
 The posts to be hooked on a forklift with a jib attachment prior to un tightened the bolts
Report all environmental incidents immediately to the foreman or
 The forklift must hold the posts during the un-tightened process directly to Environmental Team if foreman unavailable.

Change management (Has a change in Methodology / Location of work / Environmental conditions introduced any new hazards? If yes, document below and re-brief workers – changes to be updated
immediately on next revision of SWMS.)
New Hazard Control measure Responsibility

High Risk Construction Work (Does the work involve any of the following)
Risk of a person falling more than 2 metres Work on a telecommunication tower Demolition of load-bearing structure

Work in an area with movement of powered mobile plant Temporary load-bearing support structures Work on or near chemical, fuel or refrigerant lines

Work on, in or adjacent to a road, railway, shipping lane or other traffic Work on or near energised electrical installations or services Work in an area that may have a contaminated or flammable
corridor atmosphere

Work in or near a shaft or trench deeper than 1.5m or a tunnel Work in or near water or other liquid that involves a risk of drowning Work in areas with artificial extremes of temperature

Likely to involve disturbing asbestos Work on or near pressurised gas mains or piping Use of explosives

Tilt-up or precast concrete elements Work in or near a restricted space Diving work

HIGH RISK WORK Potential hazard Control measure Responsible

(from above list) Person
& WHEN APPLICABLE
Risk of person falling more Falling from heights  Inspect the scaffold access prior to use, ensure the tag is in date Workers
than 2 metres, when:  Report to area supervisor if you see any damage on the scaffold Supervisor
 Accessing the scaffold Falling objects  Compliant fixed edge protection must be in place when working above 2m. (RCPBJV and
access tower  Truck to use full railed edge protection system before any personnel access the tray of the truck Macfab)
 Using EWP  Competent and qualified operators to use EWP
 Check ground conditions prior to using EWP
 Do not exceed manufacturer’s loading requirements
 Daily check of EWP and harness to be completed prior to starting work. Ensure harness is tagged and in
date.
 Pre-start check to include sky guard activation test and use of ground override controls.

Macfab Westconnex M4 Widening SWMS
 When working from EWP ensure that harness is worn and attached to the designated anchor point at all
times
 Do not stand on handrails of machine at any time
 Do not over reach when working from a boom
 Prior to starting work, an ‘Emergency Controller’ with EWP operator’s qualification to be nominated to
bring basket down using the ground controls in the event of an emergency. This person must remain on the
ground in visual contact during EWP use at all times. If visual contact cannot be maintained a UHF radio
must be used.

Work in an area with Being trapped or struck  Plant pre-start to be completed prior to works commencing Workers

materials  Only ticketed and VOC’d plant operator and dogman
 Operating a forklift  All workers not necessary for the completion of the task to stay outside of the radius of the mobile plant
 Lifting the posts and  Set up exclusion zone for EWP and lifting operations and this is to include the area underneath the EWP and
mesh bridge during the safety screen posts and mesh installations.
 Operating EWP  Safety screen posts and mesh are to secured on the truck during the delivery
 No one shall be underneath suspended loads
 Do not slew over the workers
 When required to enter the exclusion zone of an item of plant positive communication must be made and
buckets grounded / machine isolated.
 Lift using certified lifting points
 All lifting gear (chains, slings etc.) to be inspected, tagged and in date prior to use by competent and
ticketed dogman.
 Always ensure that the load is in a safe and secure position prior to removal of the lifting accessories
 Assess wind conditions prior to lift
 All lifts must be within 75% of the crane lifting capacity
 Routine lift plan to be completed for lifts below 75% WLL
 Lifts between 75% and 90% capacity must be properly assessed and require and approved lifting plan
 Detailed work methodology to be communicated to the work group as part of the task pre-start

 Comply with SEER requirements to work with forklift:

 Detailed Vehicle Movement Plans (VMP) will be written to control the movement of other mobile
plant in the work area. This will ensure that they do not enter the forklift exclusion zone where
the dogman will be working unless called upon by the dogman.
 A reliable means of communication is established between the forklift operator, the dogman and
the surrounding mobile plant via a UHF radio.
 The dogman will remain in full view of the operator at all times as the operator is required to see
the load.
 The dogman will position themselves on the opposite side of the load to the forklift.
 Tag lines to be used to control the movement of the loads.
 Work Area Briefing: Presented to the crews during the pre-start brief. Changes to the site
conditions/procedures (if any) will be captured in the Change Management section on a
start/change card
