    Department: LD#2 SC Section: SMLP Location: LF#1 electrode platform
    Name: Uttam Kar P. No.: 129496 Age: 56 years
    Date of Incident: 16.11.21 Shift: ‘A’ T.O.I: 9:50 AM

    What happened: At around 9:50AM, while handling nipple for electrode work he was engaging nipple to
    RS north crane hook(20T hoist). During that period due to uncontrolled swing of crane CT, his left-hand
    thumb got pressed with crane hook. The IP attended First Aid from where he was referred to TMH for X
    Ray. X Ray was normal (No Bone Injury). Doctor declared him fit for duty.

    Preliminary Findings:

    • IP was wearing all required PPE’s.


    • From the preliminary investigation, uncontrolled movement of crane hook(20T hoist) was observed.

    Immediate actions taken:

    • PIR has been logged (PIR number: IN00128552) for further investigation.
    • The incident has been communicated to all the employees.
    • Brake of RS North crane 20T hoist adjusted.
