Instructions:
1. This permit shall be completed by Chief Engineer for transfers within Engine room and that
Chief engineer will be responsible for safe transfer operation & same to be manned.
2. This permit shall be completed by Chief Officer for transfers related on deck and related tanks
and that Chief officer will be responsible for safe transfer operation & same to be manned.
3. Master shall review such transfer plan, approve & sign, and forward permit for concurrence to
VM (Vessel manager) / MS (Marine Supt). Both VM and MS must be included in any such
communications.
4. Office Concurrence: VM to approve for transfers within ER. MS to approve for transfers on
deck. VM and MS to approve for transfers involving ER to/from deck. ECD in copy only.
