EXCERPTS FROM THE MINUTES OF THE FACULTY MEETING OF JONES WEST

HIGHSCHOOL formerly JONES RURAL SCHOOL-MALANNIT ANNEX, MALANNIT,
JONES, ISABELA, HELD ON DECEMBER 15, 2021 AT THE PRINCIPAL’S OFFICE.

A RESOLUTION EARNESTLY REQUESTING YOUR GOOD OFFICE FOR THE

PROVISION OF 100 PCS MONOBLOC CHAIRS AND 10 LONG TABLES FOR OUR
SCHOOL AMPITHEATER.

WHEREAS, Mrs. Juliet V. Rodavia, School Head, Faculty and staff together with the Punong
Barangays of feeder barangays (Malannit, Pungpongan, Dipangit, and Villa Victoria) agreed to
request your benevolent office for the provision of monobloc chairs and tables which can be used
in the school amphitheater especially when face to face classes commence or in events when it is
needed by the barangay and other stakeholders;

WHEREAS, the faculty members, punong barangays of the feeder barangays and PTA officers
and representatives affixed their signatures in this resolution, unanimously agreed to ask for the
assistance of your office to meet and address the demand of all the students and parents who
have strong desire to undergo the limited face to face classes;

WHEREAS, the school has a partially finished amphitheater, already functional but lacking basic
amenities like tables and chairs which are vital when gatherings are held for the purpose of
accomplishing the goals, projects and programs of the school aligned with the Department of
Education;
WHEREAS, the faculty, parents, students and other stakeholders will be more comfortable when
this basic provisions are available;

WHEREAS, the faculty members of this institution, parents, and barangay officials of the feeder
barangays agreed and approved, let it be resolved as it is hereby resolved, to request your office
for the provision of 100 pcs. monobloc chairs and 10 long tables for the school amphitheater.

We the undersigned hereby certify JONES WEST HIGHSCHOOL formerly JONES RURAL
SCHOOL-MALANNIT ANNEX’s strong desire to request your office for 100 pcs. Monobloc
chairs and 10 long tables.

CLASSROOM BUIDING;
WHEREAS, Mrs. Juliet V. Rodavia, School Head, faculty, staff together with the Punong
Barangays of feeder barangays (Malannit, Pungpongan, Dipangit, and Villa Victoria) and the
GPTA, agreed to request your benevolent office for a classroom building which will serve for
instruction specifically for the Grade 12 students under the Senior High School.

WHEREAS, the faculty members, punong barangays of the feeder barangays and PTA officers
and representatives affixed their signatures in this resolution, unanimously agreed to ask for the
assistance of your office to meet and address the demand of all the students and parents who
have strong desire to undergo the limited face to face classes;

WHEREAS, the school has recently offered its senior high school during the opening of the
School Year 2021-2022 having its Grade 11 using an extra school building which was originally
the audio-visual room/ computer laboratory of the school;
WHEREAS, for the next school year 2022-2023, Jones West High School will be offering its
Grade 12 unfortunately, enrollees will have to stay in a makeshift classroom originally a faculty
room;
WHEREAS, the faculty members of this institution, parents, barangay officials of the feeder
barangays and GPTA officers agreed and approved, let it be resolved as it is hereby resolved, to
request your office for a classroom building.

We the undersigned hereby certify JONES WEST HIGHSCHOOL formerly JONES RURAL
SCHOOL-MALANNIT ANNEX’s strong desire to request your assistance for the construction
of a classroom building.
