This document provides tables and guidelines for assessing risks at a power and water utility company. Table e defines an impact scale for occupational health and safety from minor to severe. Table f defines an impact scale for property damage from minor to severe. Table g defines a likelihood scale from remote to expected. The document explains how to calculate initial and residual risk priority numbers by cross-referencing the impact and likelihood ratings on a 5x5 risk heat map. It notes the risk assessment is proprietary information of the power company.

Calculation of Initial & Residual Risk:
The Initial and Residual risks shall be calculated as per the rating criteria detailed in tables d, e, f & g above to decide on at Risk Priority Number (RPN) from the table b. RPN is calculated as ‘RPN=Impact X Likelihood’. Use table ‘a’, the 5x5 Risk Heat Map to identify the rate for the Impact and likelihood.
