AFFIDAVIT OF ARREST

We, the SPO1 Anthony Sebastian II and PO2 Emmanuel Dela Peña, both
members of the Philippine National Police, presently assigned at the Marikina City,
Police Station, PCP-16, do hereby depose and state THAT:

01. That on about 9 o’clock in the morning September 20, 2009. I. 2 nd affiant
receive a report thru a phone call from Josephine Galang, housewife live in
51 Pipino St, Marikina, 1800 Metro Manila, that she is being harassed by her
neighbor

02. That immediately after receiving the report. We proceeded in the reported
place and that thereat, we saw a person’s Mark Anthony Lunar, Alvin
Tolentino and Josephine Galang.

03. That upon interview on Alvin Tolentino – He stated that Mark Anthony Lunar
committed a Sexual Harassment with Josephine Galang, by means of
slapping her butt and by telling Sexual acts.

04. That we invited Mark Anthony Lunar to go with us in the Police Station to
clarify things which he readily acceded.

04. That we informed Mark Anthony Lunar that he is being held for allegedly
committing Sexual Harassment R.A 7877: Anti – Sexual act and apprised him
of his Constitutional Rights as stated in the Miranda Doctrine. We then
brought and indorsed the case for proper investigation.

06. IN TRUTH TO THESE WE HEREUNTO affixed our signatures below to the

truthfulness of the foregoing.
