ACTIVITY ACCOMPLISHMENT REPORT

Name of the Activity : Second Portfolio Day and GPTA Assembly


Date : February 18, 2022
Venue : Silanga NHS Covered Court
Participants : SNHS faculty, students, parents, guests

Objectives/Agenda :
To acknowledge learners’ efforts and progress towards the accomplishment of
learning goals by showcasing student’s best practices, portfolio and projects.
To give recognition to students who have showed outstanding performance during
the second quarter period.
To provide feedback of students’ performance to parents and guardian.
Implement health and safety guidelines.
Highlights of the Activity :
Delivery of the State of the School Address (SOSA) by the school principal in the
school year 2021-2022.
General Parents and Teachers Association assembly.
Distribution of Certificate and Report Cards to different grade level.
Results/ Output
Parent-participants were grateful for the achievements of their children, they were
happy to see the outputs of the students displayed in the booth by the teachers.
Parents were also interested to ask feedback on the performances of the students. And
they were also informed for the upcoming pilot limited face to face classes of senior
high school student.
Report Cards and certificate was distributed.

A NARRATIVE REPORT ON
SECOND QUARTER PORTFOLIO DAY

The Regional portfolio day is an innovative activity under the leadership of Dr.
Uytico, pursuant to DepEd Order No, 36 s. 2016, entitled “Policy Guidelines on Awards and
Recognition for the K to 12 Basic Education Program,” and in cognizance of the evidence-
based performance evaluation which is an essential part of the learning process.

PORTFOLIO DAY is considered a Culminating Activities or performances wherein


at the end of every quarter, schools are encouraged to put up exhibits of student’s outputs
across subjects or learning areas.

Silanga National High School had its portfolio day on the 18th of February 2022.
Parents, students and other stakeholders attended the said event. Dr.Dandy Acuin, Regional
EPS- AP/Filipino, and Ma’am Hydelyn Cinco, Regional Librarian came to witness the
activity as well as the Division Monitoring Team, Ma’am Veronica G. Buenaventura-EPS
and Ma’am Carmen Lim, EPS-Science attended the event. Sir Edgar U. Dolorzo, EPS-AP
and Ma’am Antonieta Jacob, PSDS were present in the activity.

The events begin with the singing of the Philippine national anthem and followed by
an invocation and the singing of Catbalogan City Division Hymn. Portfolio Coordinator, Mr.
Martinito C. Bacaltos, formally opened the program by sharing an inspiring message to the
learners, parents and other stakeholders. Dr. Dandy Acuin, Regional EPS- AP/Filipino,
Ma’am Antonieta Jacob, PSDS give their inspirational message to the parents and students
who were present. The school principal Sir Rhum O. Bernate delivered his State of the
School Address (SOSA).

After the messages given, guests and judges were given the opportunity to observe
booth of each grade level and scan the display or outputs of the students. After the
announcement of the winner of booth, there were also tokens for the first five parents who
arrived early at the venue.

The school principal Sir Rhum O. Bernate open a forum on the issues and concern
where parents could ask questions regarding the status of their children and other school
matters. Sir Rhum O. Bernate give emphasis on the dissemination of information about
expanded limited face to face classes. Follow up questions and queries were also clarified.
Afterwards, everyone had a chance to have a glimpse of the students’ outputs at each grade
level booth thru a gallery walk.

Second part of the program followed as the parents proceeded to their respective
rooms for the PTA meeting and distribution of certificates and report cards. It is indeed
crucial to have a parent-teacher adviser conference since it’s the best way to have feed
backing of student’s performance.

This year “Portfolio Day” has been a great success and has effective served its
purpose.
