DURING QAQC MEETING

Dear Ma’am/Sir,

Good day!

We would like to inform and concern to your good office the repetitive and excessive absences of your deployed QAQC/PIC
at Callisto Tower 1. We would like to inform you the gravity of this matter since you are required to provide a QAQC/PIC
on-site as per stated on the prelims of your contract. Your QAQC/PIC will represent your company during
QMBWA and QAQC Weekly Meeting as we discuss issues and concerns on-site as well as the alignment of
Procore Utilization to RBG requirements. Attendance to our weekly QAQC meeting is for strict compliance to
your deployed personnel, their attendance will reflect as well on your quarterly Vendors Performance
Evaluation Report (VPER).

Our Procore Utilization needs to be focused on since we are audited daily by PDCs/ODT on its completeness
and alignment with the RBG requirements.

Our Procore Utilization compliance are being reported and monitored daily by ISO ODT and presented during
MANCOM meetings.

We are hoping that you take action on this.
