I would like to let you know that there are deviations of ISG safety procedures, due to
lack of supervision.
During collection of wood pallets in new military camp, I found the following safety
issues:
 The TGCTP truck operator working with expired license.
 He doesn`t have daily check list.
 The TGCTP truck is not inspected.
You are kindly requested to make a plan to ensure that all issues to be fixed ASAP.
Note : for these reasons the TGCTP truck operator stopped immediately from work.
 Every rotation, we spend a lot of time to talk with your disciplines supervisors
about the work place arrangement , but I have confirmed that there is no a
good will as well no engagement from their side to keep our workplace safe
and in good order .
Almost of time the worksite is untidy, really dirty and in a mess. That’s why We are
in urgent need to your presence in order to arrange a serious meeting in TEG site with
your disciplines supervisors to change humour and to be more involved in Safety as
their first duties.
Please have a look at the attached document.
 Please find attached the list of TGCTP & CTCO vehicles for inspection.
Would you please communicate me the final inspection report and all
anomalies; so that I can follow up the actions.
 Further to the email below, as there is no action done to repair the anomalies
mentioned above in red since the two month ago (tow monthly inspections).
 The concerned machinery is not allowed for driving till entire reparation,
except the compactors which are not under our Authority.
 I think that we have different discussed several times about the separation of
PTW when we have two activities
 I have noticed this morning during the HSE weekly presentation, that one of
your supervisors was not present.
 Be informed that the presence of supervisors is mandatory also more than
required.
So all supervisors to lead by example, for people tend to emulate their leaders
 I would like to let you know that there is a big discrepancy of non-compliance
to ISG control of work standard; due to lack of supervision. Today we have
noticed there is only site TL. You are kindly requested to make a plan to
ensure that all activities will be carried out under full supervision
 Would you please arrange a meeting this afternoon to fix the matter
definitively related to persons who work for our department as ISG staff.
 Please have a look at the attached internal environmental audit carried out
recently then proceed to close out all these findings to keep our site safe, clean
and tidy.
An inspection will be arranged to check the actions.
 I have a discussion yesterday with KBA LOGISTIC Supervisor:
Zakariaabboura, about the availability of the land transport advisor (Toufik),
and he inform that toufik may not be available in this rotation, So for that I
spoke With ISG construction manager about this issue, and he agreed that there
is no problem for Re-assessment just made all the request ready, the main issue
will be for the new drivers for Assessment, So please take this information on
Your consideration.
 Is there any action about the coordination with the maintenance supervisor to
start the above inspection of our accessories and lifting equipment .
 Gents,During a routine work fronts inspection at the OPS warehouse B, there
is a TGCTP civil crew who are working there, they are doing a mechanical
excavation, but I was surprised when I checked the PTW, I noted that there is
not TRA, and the attached excavation certificate in not filled out, also the
sketch is not marked as scanned area. And the surveyor was working there, but
his task is not including in the PTW, even him he does not know!
 , you see there are many anomalies in one PTW!!! This kind of things should
not happen, remember just a few days ago we had an incident an the aero port,
your JCB hit a water pipe when he was excavating, Coordination need to
improve within your team.
 has experienced strong opposition and challenge regarding safety items
identified at TEG by Akim TGCTP TL at TEG. I believe he is now leaving. I
have also been challenged by him too regarding safety items identified. I
believe this individuals safety beliefs , hence his attitude and behaviours are
not in line with ISG’s philosophy of No accidents , No harm to people and

 Too many comments have been added and the reference is done.be informed
that the way you are doing the TRA is totally wrong, because the TRA
preparation is not an individual HSE work, so to comply with procedures, you
have to conduct a meeting with all TGCTP team (civil supervisor and site TL)
then you send an invitation to ISG team (Area Authority, Supt and HSE Eng)
for TRA meeting or at least send the document for comments.All these
preparations should not be done in the morning of task , you need more and
enough time to do that.
 Mohamed,Your visit to TEG site is more required to give support to Soufiane
in order to clarify him the ISG procedures because now is doing his best and he
is showing good behaviour to be adhered with our system, except he needs
some guideline and an official instruction for how to do his main task.
Kellou,You were not copied in the first email by TGCTP HSE so if you have
any comments, please add theme before proceeding for the approval.
 You have in your store an expired extinguisher; it should be replaced ASAP or
reinspected.
 Further to my worksite inspection conducted this morning , I have noticed that
the backhoe ( jackhammer ) represents a high risk due to hydraulic system oil
leak which likely can result in severe and serious incident.
For that reason , You are requested to remove the stated breakers from site once
you finish the assigned task also we never accept unsafe equipment to be used on
site, because they are considered as root causes of incidents.
 Further to my worksite inspection conducted this evening , I have noticed that
TGCTP team working without vehicle certificate and the flagman stay from
away the lowboy when it working the represents a high risk due to lowboy
moving in the CC which likely can result in severe and serious accident. For
that reason you are requested :
-Take actions to avoid ISG procedure` violence.
-Ensure that this kind of things should not happen, remember that days ago we
had an incident in military camp for the manhole.(Please enough enough
enough).
 See email from Karim which was sent few days ago without any answers from
your side despite the reminders sentthe first request sent from TGCTP and
when we requested more information, nothing received from your sideTo be
professional you should be more cooperative
 Totally agree that more frequent Safety stand downs must be regularly planned
in all sites in order to allow all people at all levels understand their own roles
and responsibilities about HSE – I requested from our HSE guys (HSE Senior
&Fahim) to attend in KBA in order to give more attention and also help
TGCTP to improve HSE performance – Our HSE guys and TGCTP HSE team
should discuss and agree on the subject to discuss prior to the meeting in all
sites – that’s to help transferring knowledge and experience.I would like from
all to understand that a drop of HSE performance from one of our contractor is
the same than a drop of HSE performance for all Projects team – we all success
or we all fail – our policy is: “One team, One goal”.

Please ensure this is done regularly across all sites until the situation improves – this is very
important.

careful, attentive and concentrated when working and performing all planned
activities.

can`t be achieved unless cooperation/coordination and communication is at
required high level.

injuries or ….., you came from your families to sites and then you return to
your families again.
-My other message is also to be happy when performing any activity and to
avoid to be stressful or crispy and if anyone (in some exceptional situation
and for some reasons) is in this case, he has to see with his responsible how to
manage his time in order to return back to normal behavior and to work with
happiness and pleasure.
-The aim and objective of all these messages is to finish these TARs activities
in required HSE condition without any incident/accident/injuries or material
damaged.
-Pls to share these messages to all JV and contractors teams involved in your
different meetings.
-In one word, Our common and Individual target is and should be, during the
whole TAR period: HSE

 Because of the security situation, we remind you that we must pay special
attention to the respect of professional identification badgethe badge is a means
of identification, and therefore any attempt to prevent malicious intrusion.
 We need to change hearts and minds and that takes time and can only be
achieved via engagement & leadership which eventually changes beliefs,
attitudes and ultimately behaviours of our drivers.
 have noticed this morning during the HSE weekly presentation, that sixe of
your workers was not attend (see attached) Although several time we
discussed and an email was sent about these issue. And be informed that it is
must for everyone to attend these meeting because it’s more than mandatory.
For that, please take the necessary actions to prevent such inappropriate
behaviour.
 I’d like to let you know that after the daily inspection conducted for the vehicle
under registration number 04356-311-16 that you have moved two days ago
from KBA to TEG site has several defects ( Windscreen wiper & rare seat belt
does not function) .
So my concern is:
1/ Why do you take the relablevehilce from TEG and and you send a dective
one without any notification.
2/ Have you any clarification about this issue and why you don’t repair it in
KBA befor you send it to TEG.
Nb:You know that we are preparing for LTMS audit in TEG and you send a
faulty vehicles.
Be informed that I will never accept the defetcitvevehicles in my site , if you
need any transfert of your fleet ,you have first to repair them at your level then
you can proceed for movement.
 The treatment of the OBM using the TCC unit near the BDV and CC is
producing bad odours that several persons are complaining about. We need to
resolve this issue which is causing disagreement and influence the workers’
health.
 Because of the security situation, we remind you that we must pay special
attention to the respect of professional identification badge permanently and
apparent. The badge is a means of identification, and therefore any attempt to
prevent malicious intrusion. It is therefore necessary to take action to raise
awareness among staff of all sites (KBA, TEG, REG, HRM), so that this rule is
firmly adhered to, and all measures contributing to strengthening our security
apparatus, including searches of hand luggage, trunks, hoods and passenger
vehicles. We will continue to reiterate our call for redoubled vigilance.
Failure to comply with this provision may interfere with proper operation and synergy
required in the implementation of all measures taken and which contribute to the
security apparatus in place.
 Ismail,honestly we share the same concern, especially that we put a lot of barriers such
as daily inspection and monthly pre-inspection to avoid such remarkable observations,
So I will investigate deeply and I will send back to you a final report with the proper
action to avoid such findings.
