MENTAL HEALTH STATUS

OBSERVATIONS

 As I examined my grandfather I came to these results, for the appearance he is disheveled, or he

is untidy maybe because he only takes a bath during weekends because of his personal reasons.
When it comes to his speech, he is normal, as I approached him, he answers me directly in a
normal way. When it comes to eye contact it is normal as well because as I asked him, he looked
me in the eyes. When it comes to motor activity, he slowed because as he gets older his body
became weak and that’s normal and at last the affect, he is constricted.

MOOD

 He is anxious because he is already older person. He became anxious because there are some
factors affecting on his age.

COGNITION

 When it comes to orientation impairment he forgot person, sometimes the person who is closed
to him, he forgot to recognize. When it comes to memory impairment, it is in the short term
only because he can know some of the word or numbers that I give to him he can recognize in a
short period of time. When it comes to his attention, he is distracted because if there is a event
or certain mode of attraction in other place of our compound he is out of attention during the
mental health status exam.

PERCEPTION

 When it comes to hallucination, he is not been there as I observed and asked as well. He might
some changes the way he interacts, but it is not called a hallucination.

THOUGHTS

 When it comes to suicidality, he never thinks that thing even there is a problem, as I observed
and asked him. He is the one who gives advice to us, not to involve in this kind of mindset. When
it comes to homicidally, he never thinks that way and for the delusions he never goes that way
as well.
BEHAVIOR

 He is cooperative because as I observed and interact with my grandfather, he answer me in a

nice way even if there is some questions that he has having a hard time to answer nor to
recognize.

INSIGHT AND JUDGEMENT:

 I cam to the result of being fair because as he reached his age, he can able to be cooperative in
this kind of examination, he can recognize some of the activities that I give and he answer me
with in the nice way. Some negative results are normal because of his age as he gets older it can
affect some factors on my grandfather.

 Mr. Amado Chicano results as I examined and observed him, he can know the year, season,
month, day, date, and country as I asked him, he answer me by looking into my eyes and he is
confident. Then, as I give an activity which is repeating a word (the apple, penny, and table)
three times, he normally knows and remember these three words that I give to him. When it
comes to the spelling of a word “WORLD”, he can spell it, but he spelled it slowly but correctly.
Another one is, repeating the three word that I give in from the question three, he knew the
apple and table only, but he forgot the penny word. Next, the pointing some things and know if
he can easily identify it, I point a pen, a stone, a plastic flower, and grateful because he can
easily identify it. When it comes to repeating the phrase which is “No ifs, ends or buts”, he
cannot do that task, maybe because of his age he cannot be able to perform this one. When it
comes to the, performing stages of task, I asked him to get a paper and follow the steps of folds
that I were going to perform, he managed to do the task even he closes his eyes. I asked my
grandfather to write a sentence, so he results is that he writes a “gwapo ako”, which is a little bit
funny, but I can see that he is trying and he can be able to write. Lastly, for the diagram, he
cannot perform this kind of procedure.
