TRANSITION
Identify a plan, with support from school staff, if you are prepared to
graduate or move to/from a different school or program.

- The plan that I am making in preparation for me to graduate or to move from a different program are:

 Making sure that I did all my tasks in school to avoid some conflict before graduating or shifting
from another program.

 Making sure all the requirements, and documents are met and submitted.

 Create a schedule for dealing with some school applications to keep me on track.

REFLECTION
Student self-evaluation

- I evaluate myself as a hardworking and yet slightly sluggish student in completing my tasks but doing it
efficiently as a bonus. I also evaluate myself that keep myself on track, doing the college works completely
even if I am struggling sometimes.

Parent/Guardian Reflection

- My parent evaluates me as a sluggish one when making many assignments but they see in me the eagerness
and the balance of studying and doing some free time.

Teacher/Advisor(s) Reflection
- My professors and adviser will evaluate me as a responsible and hardworking student as I do my
assignments, activities, and cognitive tests without being incomplete but being on time in passing
those requirements is slightly a flaw for me.
