‫المقدمة ‪-:‬‬
‫الحمد هلل رب العالمٌن والصالة والسالم على سٌدنا دمحم وعلى اله الطٌبٌن الطاهرٌن وصحابته المنتحبٌن‬
‫وبعد‪,,,,‬‬
‫ٌعتبر مستشفى ‪22‬ماٌو ضالع همدان احد المؤسسات الوطنٌة التً تقدم خدماتها الطبٌة العالجٌة لشرٌحة واسعة‬
‫من المجتمع سواء من المحافظة والمناطق المجاورة ومعظم المدٌرٌات كما تعتبر الوجه االولى للمواطنٌن من‬
‫ذوي الدخل المحدود كون المستشفى ٌقدم خدماته الطبٌة بأسعار رمزٌة مقارنة بالمستشفٌات والمراكز الخاصة‬
‫وكما ان المستشفى ٌقوم بتقدٌم خدماته مجانا لجمٌع المرضى والحاالت المعدمٌن والنازحٌن والجرحى واسر‬
‫الشهداء ‪ ,‬فً الظروف الحالٌة التً تمر بها البالد بسبب الحرب الغاشمة التً تشنها دول العدوان على بالدنا‪.‬‬
‫وعلى الرغم من الظروف القاهرة الذي ٌمر بها الوطن اال ان المستشفى استطاع بفضل هللا ان ٌحقق الكثٌر من‬
‫االنجازات فً فترة وجٌزة ودعم ومساندة مكتب الصحة و الوزارة امتثاال لتوجٌهات القٌادة السٌاسٌة الحكٌمة و‬
‫التً تسعى لتطوٌر القطاع الصحً فً الوطن‪.‬‬
‫وتلبٌتا للطموحات المرجوة فان المستشفى ٌسعى جاهدا للحفاظ على الخدمات المقدمة حالٌا بالجودة المطلوبة ‪,‬‬
‫كما ٌسعى لتوفٌر الخدمات الغٌر الموجودة فً الوقت الراهن داخل المستشفى ومن تلك الخدمات توفٌر جهاز‬
‫القسطرة القلبٌة والطرفٌة لتقدٌم هذه الخدمة لمرضى القلب والسرطان والفشل الكلوي ‪ ,‬كون هذه الخدمة موجودة‬
‫بشكل ضئٌل فً المستشفٌات والمراكز الحكومٌة بشكل عام وارتفاع كلفة هذه الخدمة فً المراكز والمستشفٌات‬
‫الخاصة‪.‬‬
‫وبناء على ما سبق فان مستشفى ‪22‬ماٌو ضالع همدان ٌسر ان ٌقدم هذه الدراسة المختصرة لمعرفة الجدوى من‬
‫توفٌر هذه الخدمة داخل المستشفى‪.‬‬
‫بناء على البٌانات االحصائٌة التً تم جمعها من االطباء الذٌن ٌستقبلون الحاالت فً مركز الطوارئ العامة وعٌادة‬
‫الباطنٌة فً المستشفى بان المستشفى بحاجة ماسة لتوفٌر هذه الخدمة‪.‬‬
‫تعتبر القسطرة القلبٌة من الخدمات الطبٌة ذات التكلفة الباهظة الثمن بالنسبة للمرضى بشكل عام وذوي الدخل‬
‫المحدود بشكل خاص سواء القسطرة التشخٌصٌة او العالجٌة وخاصة فً المراكز والمستشفٌات الخاصة والذي‬
‫ٌلجئ الٌها المواطن عند الضرورة كنتٌجة حتمٌة لعدم تمكنه من الوصول الى هذه الخدمة الطبٌة فً المراكز‬
‫والمستشفٌات الحكومٌة بسبب الضغط الحاصل على هذه المراكز والمستشفٌات لمحدودٌة تواجد هذه الخدمة فٌها‬
‫وهذه الخدمة تتطلب تموٌل كبٌر خارج ارادة المستشفى المالٌة وغٌر متوفر فً وزارة الصحة لذلك تطلب االمر‬
‫تدخل قٌادة المحافظة لتوفٌره ‪.‬‬

‫بناء على البٌانات االحصائٌة والمعلومات التً تم جمعها من االطباء الذٌن ٌستمبلون الحاالت فً مركز الطوارئ‬
‫العامة وعٌادة الباطنٌة فً المستشفى والبٌانات التمدٌرٌة التالٌة‪:‬‬

‫اوال‪ :‬الجانب االحصائً‪:‬‬

‫‪ -1‬الحاالت الطارئة والحرجة‬

‫تمدر عدد الحاالت الواصلة الى لسم الطوارئ بحوالً ‪ 66‬حالة ٌومٌا منها ‪ %15‬حاالت ذات عاللات للبٌة حوالً‬
‫‪ 9‬حاالت ٌومٌا ‪ .‬اي هنان ما ٌمارب ‪4‬حاالت ٌومٌا ‪ 126‬شهرٌا اي ما ٌمارب ‪ 1446‬سنوٌا بحاجة الى اجراء‬
‫تدخل تشخٌصٌة او عالجٌة فً مركز لسطرة للبٌة ‪.‬‬
‫‪ -2‬الحاالت الغٌر طارئة‬
‫ٌتم تحوٌل حاالت غٌر طارئة وهً من عٌادة الباطنٌة والعناٌة المركزة والرلود اكثر من ثالث حاالت ٌومٌا بمعدل‬
‫‪ 96‬حالة شهرٌة ‪ 1606‬سنوٌا وعند الولوف البٌانات االحصائٌة السابمة نجد ان عدد الحاالت التً تحتاج الى لسطرة‬
‫للبٌة ٌساوي (‪ 2526 =)1606+1446‬حالة سنوٌا‪.‬‬
‫‪ -‬نسبة الحاالت التً تحتاج دعامات للبٌة عالجٌة تمدر ‪ %66‬من اجمالً عدد الحاالت المحولة لعمل لسطرة‬
‫للبٌة (‪ )2526‬حالة اي ما ٌساوي ‪ 1512‬حالة سنوٌا من اجمالً الحاالت التً عمل لها لسطرة تشخٌصٌة‬
‫‪ -‬عدد الحاالت التً ٌعمل لها لسطرة تشخٌصٌة بنسبة ‪%166‬من اجمالً عدد الحاالت المحولة لعمل لسطرة للبٌة‬
‫ما ٌساوي ‪ 2526‬حالة سنوٌا ‪.‬‬

‫ثانٌا‪ :‬الجانب المالً‬

‫تعتبر المسطرة الملبٌة من الخدمات الطبٌة ذات التكلفة الباهظة الثمن بالنسبة للمرضى بشكل عام وذوي الدخل المحدود‬
‫بشكل خاص سواء المسطرة التشخٌصٌة او العالجٌة وخاصة فً المراكز والمستشفٌات الخاصة والذي ٌلجئ الٌها‬
‫المواطن عند الضرورة كنتٌجة حتمٌة لعدم تمكنه من الوصول الى هذه الخدمٌة الطبٌة فً المراكز والمستشفٌات‬
‫الحكومٌة بسبب الضغط الحاصل على هذه المراكز والمستشفٌات لمحدودٌة تواجد هذه الخدمة فٌها‪.‬‬
‫وٌمكن توضٌح التكلفة التً ٌتحملها المرضى كما ٌلً‪:‬‬
‫باحتساب متوسط كلفة اجراء القسطرة التشخٌصٌة والذي ٌساوي ‪252‬دوالر فً عدد الحاالت ‪$ 602,222 =2522* $252‬‬ ‫‪-‬‬
‫اما بالنسبة لمتوسط كلفة اجراء قسطرة عالجٌة (تركٌب دعامات) والذي ٌساوي ‪$ 4022,222 = 0442* $0222 $0222‬‬ ‫‪-‬‬
‫اجمالً ما ٌنفمه المرضى المحولٌن من المستشفى بسبب عدم وجود لسطرة للبٌة = ‪ $ 4,956,666 =4,326,666+636,666‬اربعة‬
‫ملٌون وتسع مائة وخمسون الف دوالر سنوٌا اي ما ٌعادل بالٌمنً ‪ 2,9,6,666,666‬اثنٌن ملٌار وتسع مائة وسبعون ملٌون لاير ٌمنً‬
‫سنوٌا‪.‬‬

‫ثالثا‪ :‬جدوى توفٌر خدمة القسطرة القلبٌة فً المستشفى‬

‫‪ -‬فً حالة توفر لسطرة فً المستشفى فان ذلن سٌوفر ما ٌمارب ‪ %,6‬من الكلفة الذي ٌتحملها المرٌض الٌمنً والتً‬
‫تمدر بحوالً ‪ 2,6,9,666 = 6,,6 * 2,9,6,666‬لاير ٌمنً سنوٌا‬
‫‪ -‬فً حالة توفر لسطرة للبٌة فً المستشفى فات ذلن ٌحمك اٌرادات جدٌدة فً المستشفى تمدر ‪ %36‬من الكلفة التً‬
‫ٌنفمها المرٌض الٌمنً اي ما ٌساوي ‪ 091,666,666 = %36 * 2,9,6,666‬لاير ٌمنً سنوٌا حٌث سٌساهم هذه‬
‫االٌرادات فً رفد المستشفى بسٌوله مالٌة ٌستطٌع من خاللها تحسٌن وضع كادر المستشفى وتوفٌر االحتٌاجات‬
‫والمستلزمات وتغطٌة العجز المالً الحالً وكثٌر من نماط التحسٌن ‪.‬‬

‫رابعا‪ :‬التكلفة التقدٌرٌة ألنشاء المشروع‬

‫تمدر الكلفة التمدٌرٌة لتوفٌر لسطرة للبٌة فً المستشفى بحوالً ‪ 356,666‬ثالث مئة وخمسون الف دوالر للمسطرة‬
‫ذات جودة عالٌة (مجددة مصنعً فً الشركة المصنعة ) وحوالً ‪ 1,166,666‬ملٌون ومائة الف دوالر لمسطرة‬
‫جدٌدة ذات جودة عالٌة اوربٌة او امرٌكٌة الصنع وبحسب المواصفات الفنٌة الملحمة ‪.‬‬

Specifications
ITEM NO QTY

1 purpose:
Dedicated flat panel detector single plane angiography system for
interventional
cardiovascular procedures.
Operational Requirements with minimum specification:
State of the art, single plane floor or ceiling mounted C-Arm
cardiovascular digital
imaging system with high resolution flat panel detector technology for
diagnostic
procedures and interventional cardiovascular procedures e.g. coronary
angioplasty,
balloon valvuloplasty, vascular Angiography, online DSA etc. with
following capabilities:
· Should be capable of real time digital angiography acquisition.
· Should be capable of road mapping with zoom, freeze frame and
advanced facilities.
· Should be capable of storing fluoroscopy / cine sequences on hard disk
and GD for review.
· Should be capable of head to toe patient coverage without changing
position of the patient.
· Should be user-friendly for quick access and full control of all
functionality within the examination room.
· The system must include all packages for Cardiac applications.
System should be complete with pressure injector, Hemodynamics.

2 C-Arm Multi-directional
C-Arm Multi-directional, should be ceiling-mounted or floor
mounted with equivalent maneuverability for unobstructed
resuscitation during cardiac arrest, while continuing to do fluro
and/or cine at various angulations without any obstruction at the head
end.
should be capable of performing coronary angiography and coronary
angioplasty and balloon mitral valve and other cardio logical
interventions
All movements should be motorized with C-Arm angulations of
minimum RAO/LAO +105 deg. / -105 deg. CRAN/CAUD +45/-45. at
head end position; with 20 deg. / sec. or more speed for LAO/RAO and
15 deg./sec or more speed for CRAN/CAUD.
All movements should be in 1 steps. In addition, motorized movement
of the detector on the vertical axis at specified speed must be available.

Motorized peripheral position for peripheral and vascular

intervention should be available.
System should be capable of doing head to toe coverage without
repositioning the patient.
The C arm should have auto-collision protection with patient & the
table
Facility for motorized / manual positioning / rotation of the stand /
ceiling pivot to enable left side, right side and head side imaging for
improved workflow and for ease of operation from both left and right
side of the patient in addition to zero degree normal head end position.
Patient access should be possible from either right side or left side
Isocentre to floor distance for frontal C arm should be at least 100 cm
or more.

All movements of the gantry should be controllable from the table side.

Gantry depth should be more than 90 cm for better groin access

automatically adjust the dose according to the size of the patient

Should have minimum Radiographic range 40-125 KVp

Should have automatic exposure control for all modes of operation.

X-Ray tube should have at least 2 focal spots (larger not more than
1mm) .

The X-ray tube should have high cooling rate with liquid bearing
technology or equivalent for continuous and noiseless operation and
capable of pulsed fluoroscopy on both focal spots.
The X-Ray tube should have anode heat storage capacity of more than
3 MHU

interventional procedures
5 Flat Detector System
(i) The flat detector should be with a diagonal size of at least 25cm X
25 cm with a pixel size of not more than 180μm. The smaller pixel
size will be preferred. At least three zoom steps to be provided.
Digital system with acquisition and processing in 1024 X 1024
matrix at different fps in both fluoro and digital cine modes.
Digital fluoro loop store/replay facility & Last image hold during
fluoroscopy.
Complete cardiovascular computation software package including
clinically validated coronary, ventricular software packages (QCA,
LVA), Algorithm / software for real time stent visualization should be
possible. An easy to operate rapid calculation software for offline
coronary quantification should be available.
Facility for storage at high, medium or low fluoro
Facility for side-by-side still image; road map facility should be
provided so as to support all anatomical areas and all interventional
procedures with facility to overlay selected reference image with
fluoroscopy
Dedicated keys / touch pad for review/zoom, play/pause previous
/next image, store /recall reference images at the table side.
There should be facility to enter the patient demographics from the
examination room or the console room
The full system should have table side control operation for complete
acquisition and post processing capabilities.
Selection of stent enhancement and DSA should be possible from the
examination room
The system should have on-line & off-line coronary and left
ventricular analysis program.
The software should have Auto calibration facility for stenosis
measurement with edge enhancement and geometrical and

The full system should have touch screen control at table side

The system should have on-line DSA of excellent quality with bolus –
chase facility with motorized table movement which can be manually
controlled.
Subsequent PACS connectivity should be possible without any
additional hardware / software requirement. Auto image transfer
facility to PACS should be possible in background mode.
The complete digital system should be networked and connected to a
DICOM compatible camera.
Should have stent enhancement tool with fade in/fade out facility with
all software, hardware, image processing tools for enhancing
visualization of the stent and vessel and should be the latest and most
technologically advanced version and capable of placing on a separate
screen.
6 Monitors
The examination room should have 4 medical grade, high resolution 4
monitors of 19” or more for live, reference, stent enhancement images
and hemodynamic monitoring.
Monitors should be ceiling mounted with capability of sliding to view
from left and right of the patients.
Facility for sharing some of the monitors for display of stent
enhancement images, IVUS, FFR and EP should be possible.
One high resolution medical grade TFT/LCD monitor for post-
processing and reporting in the console room.

Control room: One / two LCD/TFT monitors for data and image
viewing. Brightness should be at least 500 Cd/m2. These monitors
should have the facility for all review post processing and
quantification of coronary and ventricular function for training and
teaching.
Another monitor in the console room for live scenes
7.00 Console

Facility for acquired images to be transferred to the workstation

seamlessly without interrupting the procedure; there should be 2 way
digital image communication between the workstation and the
procedure room.
Should be able to work with the workstation for review of the
previously transferred scenes of same patients or other patients while
procedure is going on without interruption.
Work station should be able to archive at least 1000 patients’ data
with easy irretrievability search by name, date of procedure or cath
number
The system should be able to perform QCA facility from the
examination room as well as console / workstation and facility to
transfer the scenes from archive to the procedure room. Full
quantitative analysis package should be provided.
QCA facility should be also available for recorded CD/DVD.
There should be facility to edit and delete selected scenes archived in
the work station
On CD/DVD with embedded software for reading, with facility for
zoom in and out.
FDA approved system for recording images on DVD_R/ CD_R with
DICOM Viewer in DICOM 3 format having capability of receive and

‫االسباب اخلاصت جبعل املناقصت مناقصت حمدودة باملستشفى‬

‫‪ ‬عدم توفر المبلغ كامال حٌث سٌموم المستشفى بتمسٌط بمٌة المبلغ من إٌراداته الشهرٌة وذلن‬
‫بعد االتفاق مع الشركات على الٌة السداد واولات السداد‬
‫‪ ‬فً حالة كانت المنالصة عامة سٌكون من الصعب تحدٌد مبالغ التمسٌط ضمن الشروط حٌث‬
‫ان كل شركة ستمدم عرضها بسعرها الخاص بها وبعد ان ٌتم التحلٌل والترسٌة ستنسحب‬
‫معظم الشركات اذا لم تعلم بان بمٌة المبالغ بالتمسٌط مما سٌضطرنا الى الغاء المنالصة‬
‫واعادتها من جدٌد‬
‫‪ ‬عدم ثبات االسعار مما ٌجعل الشركات ترفض الدخول فً المنالصات الحكومٌة‬
‫‪ ‬عدم اعتماد موازنة الباب الرابع االستثماري وعدم الرار موازنة عامة للدولة مما ٌؤكد عدم‬
‫امكانٌة المنالصة‬
‫‪ ‬وجود مدٌونٌات لعدد من الشركات لم تمم وزارة المالٌة بدفعها والخاصة باجهزة تم تورٌدها‬
‫فً عام ‪2614-2613‬م ومنها جهاز الممطعٌة لمستشفى ‪ 26‬سبتمبر – متنة والتً لامت‬
‫بتورٌدة شركة ناتكو وعدد ‪ 16‬اجهزة اشعة وتجهٌزات اخرى لعدة مستشفٌات بالمحافظة و‬
‫مولدات كهربائٌة لشركة االحلسً لم ٌتم دفع لٌمتها الى االن مما ٌزٌد المخاوف لدى‬
‫الشركات من الدخول فً منالصات جدٌدة‬
‫‪ ‬ارتفاع اسعار بعض الشركات ومحدودٌة المبلغ الخاص بالمسطرة حٌث سٌصعب على‬
‫المستشفى من االلتزام بالسداد بالولت والمبلغ المطلوب‬
‫‪ ‬اذا علمت معظم الشركات ان الشراء سٌكون بالتمسٌط فان معظم الشركات لن تتمدم‬
‫بالمنالصة‬
‫‪ٌ ‬جب ان ٌتم صنع ثمة بٌن الشركات المستهدفة والمستشفى بحٌث ٌتم توضٌح الٌة السداد‬
‫وطرٌمة دفع بمٌة المبلغ من لبل المستشفى مما ٌكون صورة واضحة امام الشركات المتمدمة‬
‫للمبول بتمسٌط بمٌة المبلغ‬
