Health Triangle Assessment Worksheet

The health triangle assessment will evaluate each aspect of the health triangle (Physical, mental, and
social). This will allow you to identify which areas you are lacking and which areas you are doing well.

This assessment will allow you to analyze your personal health triangle and each of the aspects that
contribute to your overall health

5. How healthy are you? Evaluate your physical, mental/emotional and social health.

I think I am healthy enough, but my physical health is not so good because I think I have sedentary
lifestyle during this pandemic other than that I think my mental, emotional, and social health is good.

6. Write a plan for improving your total health. Include details about steps you can take to improve all
sides of your health triangle.

My plan is simple, for my physical health I will just exercise regularly and eat right amount of food and
get plenty of sleep. For my mental I will just accept my flaws and imperfection and lastly for my social
health I will continue to be in touch with my friends and family.
