What Falling Accident (Contractor fell off from a 40 feet tree)

Who Novatech (Globe Sub Contractor)
Incident Details At around 3:34 PM, Amara PM received a call from Amara Detachment Commander that there is a
falling incident that happened at the Picnic Grove at Amara Phase 3A.

As per the Detachment commander, a contractor of Novatech fell from a tree (around 40 feet) at
around 3:00 PM while doing unauthorized cutting of the tree branches.

Based on the initial investigation of the team, the said contractor worked without securing proper work
permit (Tree Cutting Permit) due to pending requirements (Construction Bond, Memorandum of
Conformity on commercial terms, DOH Health Certificate and Notice to Proceed)

The said contractor was then sent immediately to Menderro hospital at around 4:10 PM but the facility
refused as they do not have the capability to treat such injuries. The patient was then forwarded to
Eversly at around 4:30 PM and was treated immediately, however the patient was then declared by the
doctor dead while on treatment at around 5:30 PM.
Response Taken 1. Amara DC immediately informed Amara PM of the incident
2. Amara PM escalated the incident to APMC VMG OM (RBG)
3. Patient immediately rushed to Menderro Hospital at around 4:10 PM but was refused due to
limited capability
4. Patient transferred immediately to Eversly Hospital at around 4:30 PM
