1.0 SCOPE OF WORK

1.1 General Description


This Method Statement (MS) is to describe the methodology for mock-up span
works and other related work within the Projek Mass Rapid Transit Lembah
Kelang Sungai Buloh – Serdang – Putrajaya (Package V202).

This MS is to be read in conjunction with reference documents listed in Clause


1.2. or documents stated in specific clauses in this MS.

1.5.1.1. Construction Manager


a) To control, manage and monitor site operation activity including staff,
machineries, related activities and any issues arise within the site.

b) To ensure mock-up span works are completed safely and on time.

1.5.1.2. Project Engineer


a) To prepare method statement and SOP.
b) To coordinate with the subcontractor that they are following the construction
drawings and proper procedure as per the requirements.
c) To liaise with SC for handling the technical matters that may arise during
mock-up span works.

1.5.1.3. Site Supervisor


a) To check and ensure all equipment is working properly before start
operation.
b) To supervise all activity in mock-up span works.

1.5.1.4. Survey Team


a) Surveyor to perform survey work before and after construction work assisted
by chainman.

1.5.1.5. Skilled Workers


a) To execute segment launching operation and post-tensioning works.

1.6 Construction Procedure

1.6.1 Survey and Setting out


a) Setting out and survey works mainly consist of setting correct structural
vertical and horizontal alignments, dimension and required levels as
specified in relevant construction drawings.
b) All survey works must refer to approved control point and bench marks
which must be either obtained or set out according to approved construction
drawings.
c) All survey datum and reference shall be using approved datum and reference
only and the Joint Survey shall be conducted by SC.
d) Potentially affected stakeholders are to be notified via Notice Mail Drops 5 days
in advance. Content of the Notice must contain information on the works
duration, hours, affected area and contact details (person, company and mobile
tel) for public to contact for information or to complaint.
e) All lane or road closure that is required as part of the TMP requires PDP PR’s
consultation. Should a traffic jam be anticipated from the lane or road closure
especially it is located at a high traffic area, a Traffic
Announcement/Construction Notice is needed. SOP of Traffic Announcement is
listed in the PR and Stakeholder Manual (Ref SSP-PDP2-GENE-OVPR-MAN-
000002).

1.6.2 Footing Foundation Construction by SPC (Design by WPC)


a) Plate Bearing Test will be conducted as requested by SC at the designated

area prior to construct footing.


b) The area of the temporary footing foundation shall be excavated to remove
any soft earth and debris. The top level of the footing shall be at least
150mm higher than the surrounding ground level (Figure 1.6.3 (a)).
c) The level chosen shall be flat and not inclined or sloping. The location shall
be free of flooding and ensure no overflowing of top of footing. SPC to check
that sufficient drainage channel away the rain water from the area.
d) The excavated ground shall be well compacted. Next, 200mm thick layer of
crusher run shall be laid and compacted. This is followed by 50mm thick
Grade 15 lean concrete.
e) Outer formwork shall be fixed.
f) Reinforcement shall be fixed with correct concrete cover.
g) Inserts (if any) to secure or brace the segments shall be installed.
h) Level control markers shall be installed to ensure correct finished concrete
level (Figure 1.6.3 (f), Figure 1.6.3 (g), Figure 1.6.3 (j) and Figure 1.6.3 (k)).
i) Grade 35 concrete shall be cast and compacted. Ensure finish top level is
flat.
j) To carry out cube compressive test for 7 and 28 days.
k) The segments shall be launched onto the footing foundation after the
concrete achieves minimum compressive cube strength of 35MPa.

1.6.3 Installation of Jacks


a) Four (4) numbers of 100 ton flat jacks (with adjustable jack bases) are
positioned on top of UC steel beam to support each typical segment during
segment launching (Figure 1.6.3 (b)).
b) At bottom of flat jack, a sliding plate (Teflon) is installed to allow longitudinal
movement due to closure of shear keys and elastic shortening during tendons
stressing.

c) At each end of the span, the pier segment will be supported by two (2) numbers
of 250 ton flat jacks (with adjustable jack bases) and two (2) numbers of 100 ton
flat jacks (with adjustable jack bases) which are placed on top of the UC steel
beam (Figure 1.6.3 (a) and Figure 1.6.3 (b)).
d) The 250 ton flat jacks are located at the same location of the permanent
bearings location. They are equipped with sliding jack bases to allow
longitudinal movement during tendons stressing. The position of these flat jack
must follow as per stated in the construction drawing.
e) The UC steel beam will be equipped with 28 nos beam clamp to ensure the UC
steel beam is firm (Figure 1.6.3 (h), Figure 1.6.3 (i), Figure 1.6.3 (l) and Figure
1.6.3 (m)).
f) Steel frame is used to strut the first pier segment for stability during erection.
g) Vertical access is placed at the side of pier segment.

1.6.4 Erection of Pier Segment


a) Prior to lifting operation, the launching team shall check and inspect all tools
and machinery including walkie-talkie, lifting wire ropes, machineries are in
good condition. Safety toolbox shall be conducted by AZSB and SPC Safety
Team before the lifting operation.
b) 160t mobile crane will be used for the lifting operation. Please refer to Lifting
Operation Permit and Lifting Operation Plan for the details of lifting plan and
rigging gears (Appendix 5.10 and Appendix 5.11).
c) As a precaution, all jacks must be locked using adjustable screw nuts to
ensure the flat jacks not moving during load transfer from mobile crane onto
flat jacks.
d) Segment erection to start with pier segment on the non-stressing end.
e) The lifting beam (spreader beam) is installed on the pier segment.
f) Connect the mobile crane lifting hook to the lifting beam.
g) The pier segment is then slowly lifted up above foundation footing (Figure
1.6.4 (a)).
h) By using the vertical access, worker can safely access onto the deck slab of
the pier segment (Figure 1.6.4 (b)).

i) Adjust the segment top level to horizontal level with spirit level and chain
block attached to lifting beam and mobile crane lifting hook (Figure 1.6.4
(c)).

j) Once pier segment is in target position and level, the two 250 ton flat jacks
and two 100 ton flat jacks are jacked up gently to bring into contact with the
soffit of the segment by applying nominal pressure, but not to push up the
segment.
k) The mobile crane then gradually releases the force to transfer the load onto
the jacks. Monitor the pressure gauge of each jack. The increase in pressure
for the jacks will indicate the load of the segment being transferred from the
mobile crane to the jacks.
l) The Surveyor shall take levels, position and alignment of the pier segment.
m) The survey information is used to adjust the segment using the flat jacks
with adjustable jack bases. After resurvey and confirmed by Surveyor, the
lock nuts of the jacks shall be turned up to mechanically lock the jacks.

1.6.5 Erection of Subsequent Segments


a) The lifting beam (spreader beam) is installed on each segment.
b) Connect the mobile crane lifting hook to the lifting beam. The segment is
then slowly lifted up above foundation footing (Figure 1.6.5 (a)).
c) Gradually bring the segment towards the pier segment. Hardwood timber
shall be used as spacers to avoid any damage/spalling to either segment.
(Figure 1.6.5 (b)). CAUTION : Recheck the direction of segment – whether
up chainage or down chainage. It will be disastrous if the segment is erected
in the opposite direction. Then the shear keys will be crushed during
stressing.

d) Segment is lifted close to the final position. Two number of temporary tie
(lever block) installed at top and bottom of segment are used to secure the
segment to the erected segment (Figure 1.6.5 (c)). Adjust the segment top
level to horizontal level with spirit level and chain blocks attached to lifting
beam and mobile crane lifting hook.
e) The 100 ton flat jacks (C) sliding base must be pre-set at least 40mm to
50mm clearance to allow horizontal movement during jointing process
(Figure 1.6.5 (e)). Next, the 100 ton flat jacks (B and C) are then brought into

contact with the soffit of the segment by applying nominal pressure, but not
to push up the segment (Figure 1.6.5 (f)).
f) The mobile crane then gradually releases and transfers the load onto the flat
jacks (Figure 1.6.5 (g)). Monitor the pressure gauges of each jack. The
increase in pressure for the jacks will indicate the load of the segment being
transferred from the mobile crane to the jacks. The segment jointing shall be
carried out by sliding the segment. The screw nuts securing the 100 ton flat
jacks (C) must be release before sliding the segment (Figure 1.6.5 (h)).
Segment shall be slide by using 30 ton Baby Jack (Figure 1.6.5 (i) and
Figure 1.6.5 (j)). WPC must ensure there is no gap between segments. Gap
between segments will affect elongation of the tendons stressing. Release
the mobile crane hook from the lifting beam. Sliding mechanism is then
being removed and shall be used for subsequent segment.
g) Surveyor rechecks the levels while jacks pistons were jacked up or down
until correct levels is achieved. Turn up the lock nuts to mechanically lock
the jacks. Then release the hydraulic pressures in the jacks.
h) Repeat steps a to g for the erection of next segment until all remaining
segments for the span are completed. Recheck the direction of each
segment to be launched before launching.
i) For the last pier segment, it will rest on two number of 250 ton and two
number of 100 ton flat jacks. The last pier segment is equipped with
instrumentation. Thus, care should be taken to ensure no damaged to any of
the instrumentation cabling during erection and stressing works.

1.6.6 Stressing Tendons


a) After segments launching is completed, the inner trumpets are installed into
the anchorage. The HDPE pipes are installed through the deviator
bellmouth shaped holes.
b) 15.7mm diameter strands with UTS 279kN are threaded into the HDPE
pipes. Please ensure there is sufficient length for stressing end (1300mm)
and de-tensioning end (600mm). This extra length shall be properly wrapped
to avoid corrosion and also to prevent it from becoming safety hazards.
c) Isolation plates are installed on to the anchorage bearing plates. Anchor
heads shall be installed on top of Isolation plates. The strands are to be
locked with wedges.
d) Before the commencement of stressing works, all the adjustable screw nuts
must be released to allow longitudinal movement during stressing works.
e) Two number of 480 ton jacks are installed by a gantry/mobile crane onto of
the pier segments (stressing end).
f) A warning sign ‘DANGER, STRESSING IN PROGRESS, KEEP CLEAR’ or
equivalent shall be displayed to warn others to keep away from the stressing
area.
g) The area at the stressing area shall be cordoned off to prevent entry by
others not involved with the works.
h) Tendons shall be stressed according to the required forces and sequences
as shown in the construction drawing.
i) The theoretical extension shall be modified based on actual Young’s
Modulus (E) and Area (A) from the strand supplier’s Mill Certificate.

j) Calibration certificates for the stressing jacks and pressure gauges shall be
provided prior to the commencement of the stressing operations.
k) No person shall be allowed to stand behind the stressing jack during
stressing in case of accidental breakage of strands which may cause injury.
l) For Double Track Span DS39.8, tendons T7 shall be stressed inside the box
girder. A stressing jack shall be lifted and brought into the box girder by
trolley. It is then hung by chain blocks (Figure 1.6.6 (a)).
m) Due to space constraint, tendons T7 to be stressed using single stressing
jack where the stressing sequence should follow as per construction drawing
reference no. SSP-H-AZSB-PKG2-OVPR-CNS-VSR-000311. Tendons T7 to
be stressed in stages as follow;

1. Stress T7 L to 33% of jacking force.


2. Stress T7 R to 67% of jacking force.
3. Stress T7 L to 100% of jacking force.
4. Stress T7 R to 100% of jacking force.

n) A nominal jacking pressure of 500psi to 1000psi or not more than 20% of


total jacking pressure shall be applied in order to take up the slack in the
tendon. The measurement of tendons extensions shall be done by using a
steel ruler. These are recorded in the stressing record form.
o) The jacking pressure is increased by increment 500psi to 1000psi or not
more than 25% of total jacking pressure and the corresponding extension is
recorded. The process is repeated until full jacking pressure is reached.
p) The extension from 0% to 25% shall be extrapolated from the measured
extension from 25% to the final jacking pressure to obtain the total tendon
extension.
q) The percentage difference from the theoretical extension shall be worked
out and compared with the allowable tolerance, ± 5%, as per stated in
Material and Workmanship Specification reference no. SSP-MRTC-GENE-
OVPR-SPC-03232, Clause 232.5.5.
r) When percentage differences than is more than +5%, check the stressing
records, whether extension linear with pressure and calculated total
extension and inform engineer of the results for further action.
s) Apply the specified prestressing force and if extension is between – 5% to –
10%. Recheck stressing records, whether linear and calculation of total
extension and percentage difference. Prestressing force shall be increased
but not exceeding 80% UTS to bring back the negative percentage within
the allowable ± 5%. If result is still out of tolerance, inform engineer on the
next course of action. Once the stressing works are approved by the SC,
wrapped the strands to protect against corrosion; or otherwise as instructed
by the SC.
t) After completely launched and stressed Double Track span, the 100 ton flat
jacks under the intermediate segment shall be removed and reinstalled for
the launching of Single Track span (Figure 1.6.3 (j) to Figure 1.6.3 (m)).

u) The sequence of launching could be changed to launching Single Track first


and followed by Double Track, depending on the readiness of site situation.

1.6.7 Monitoring
a) After completion of tendons stressing, the entire span shall be left
undisturbed for one month for monitoring purposes. The monitoring
processes include deflection of span, coefficient of losses, elongation and
concrete stress.
b) As the footing foundation is not supported on piles, there will be a little
settlement of the footings due to consolidation and compression of layers
underneath the footing. For monitoring of levels and deflection, the net
movement after deducting settlement effects at the end footings shall be
considered.
c) There is the option of jacking up the flat jacks to bring back the levels to
levels immediately after stressing.

1.6.8 De-Tensioning Tendons


a) Prior to the de-tensioning tendons, all the 48 nos 100 ton flat jacks shall be
re-installed and re-arranged according to its initial position to support the
intermediate segment.
b) The 100 ton flat jacks are jacked up gently to bring into contact with the soffit
of the segment and hydraulically locked equivalent to 75% to 100% of the
weight of individual segment. Lock the 100 ton flat jacks using lock nut by
having a 5mm gap (between jack body and lock nut) to allow vertical
movement.
c) The de-tensioning tendons shall be carried out by reversing the tendons
stressing sequence. Due to the de-tensioning of tendons, the entire span will
automatically sagging, loading from the span will uniformly transfer onto
these flats jacks.
d) De-tension the force in the tendon from one end, say the non-stressing end.
Fix a de-tensioning chair onto the bearing plate casting.

e) Take safety precaution by fixing plywood barrier at both ends of tendon,


facing the anchorage. If there were any breakage of strand, the strand shall
be stopped by the plywood barrier.
f) Using a monojack, stress a strand gradually until the strand is lifted off the
anchor head, that is when the wedges become loose.
g) De-tensioning shall be in pairs, left and right. At any time the unbalanced
between the left and right tendon pair shall not exceed more than one strand
different. For example de-tensioning of tendon T7;

1. De-tensioned T7 L – 1 strand.
2. De-tensioned T7 R – 2 strands.
3. De-tensioned T7 L – 2 strands.
4. De-tensioned T7 R – strands, and so on until completion.

h) Use a hook to pull out and remove the wedges from the anchor head.
i) Release the force in the strand to zero.
j) Repeat the de-tensioning cycle to each strand one by one. The de-
tensioning sequence shall be in the reverse order of stressing tendons.

1.6.9 Dismantling-Off Span


a) Dismantling-off span shall be carried out by reversing the segment jointing
sequence.
b) The 100 ton flat jacks (C) sliding base must be pre-set at least 40mm to
50mm clearance to allow horizontal movement during dismantling process
(Figure 1.6.9 (a)).Next, the 100 ton flat jacks (A) are then brought into
contact with the soffit of the segment by applying nominal pressure, but not
to push up the segment (Figure 1.6.9 (b)).

c) Gradually releases and transfers the load from 100 ton flat jacks (B) to the
100 ton flat jacks (A) (Figure 1.6.9 (b)). The segment dismantling shall be
carried out by sliding the segment. The screw nuts securing the 100 ton flat
jacks (C) must be release before sliding the segment (Figure 1.6.9 (c)).
Segment shall be dismantled by using 30 ton Baby Jack (Figure 1.6.9 (d)
and Figure 1.6.9 (e)).

Safety and health implementation at work site are to be exercised in accordance
with Project Safety Plan Ref. No. SSP-AZSB-PKG2-OVPR-PLN-00002and other
relevant documents stated in Cl. 1.2.

Hazards, risk and associated control measures associated with activities stated
in this MS are detailed in the Job Safety Analysis (JSA) as per Appendix 5.2.

2.2 Emergency Arrangements

Emergency arrangements at work site are to be exercised in accordance with


Site Emergency Preparedness & Response Plan Ref. No. SSP-AZSB-PKG2-
OVPR-PLN-00001.

3.0 ENVIRONMENTAL ISSUES

3.1 Implementation and Monitoring


All activities at work site shall comply with requirements stated in Environmental
Management Plan Ref. No. SSP-AZSB-PKG2-OVPR-PLN-00003.
.
3.2 Control Measures at Each Work Site
Work site shall observe; potential environmental impacts that will be generated
from activities stated in this MS, proposed control measures, responsible parties’
in-charge and relevant legal applicability. Guidelines of this mitigation are
tabulated in Environmental Aspect & Impact Register as per Appendix 5.3.

4.0 QUALITY ASSURANCE & QUALITY CONTROL

4.1 Contractual Requirements


Shall be implemented in accordance with submitted comprehensive Project
Quality Plan Ref. No. SSP-AZSB-PKG2-OVPR-PLN-00008.

4.2 Inspection and Testing


Shall refer to our Project Quality Plan Section 4.0 Project Operating Policy,
subsection 4.5.1 Inspection and Testing and 4.5.2 Inspection and Test Status.
Please refer to Appendix 5.4 for Inspection & Test Plan and Appendix 5.5 for

RFIs and respective Permit to Work shall be submitted prior execution of any
