    This document is a self-assessment form for a student to evaluate their own performance in an English class over the first school term. It consists of 5 criteria that the student rates themselves on from 0 to 1: 1) attendance and punctuality, 2) attitude and behavior, 3) accomplishment and responsibility, 4) creativity and quality, and 5) learning and knowledge contribution.     Self-assessment form

    Instructions: This self-assessment is 5 points worth. Evaluate yourself (from 0 to 1) in every of the following 5 aspects according to
    your performance in the English class during the First school term 2020-2021:

    1) Attendance and punctuality. Did you attend all of your classes on time? Subtract at least 0.5 for each absence you had during the term. If you
    were late 5 mins or more in each class, subtract the points according to your opinion.

    2) Attitude and behavior. Were you enthusiastic during your classes? ¿How was the rapport with your classmates and your teacher. If your attitude
    and behavior were not excellent, subtract at least 0.5 from this item.

    3) Accomplishment and responsibility. Did you carry out the activities assigned by your teacher during your classes very well? Did you deliver all
    your homework? Did you wear your school uniform accordingly? Did you use electronic devices during class? If you missed any of these, subtract at
    least .5 from this item

    4) Creativity and quality. Did you contribute meaningful ideas during your classes? Did you do all your tasks in a neat and error-free way? If you
    were not creative enough and made mistakes in your tasks assigned by your teacher, subtract at least 0.5 from this item.

    5) Learning and knowledge contribution. How much did you learn and how much knowledge did you contribute during your classes when
    working in pairs and/or in teams? If you did not learn accordingly and did not contribute ideas while working in pairs or in teams, subtract at least 0.5
    to this item.

    Assign your points that you think you deserve for each of the items above in the following table. Then add up your points and write the total.
