Level 2 HSE walkthrough –Borouge Retubing (brian , Majid, Naseer, Ansil, NIBU – 07/02/2023)
With our general manager and project manager, a walkthrough of borouge retubing was conducted.
Everything was found to be in order during our HSE walkthrough. The observations made during the
walkthrough are listed below.

The permit to work system should govern all work operations, and rigorous adherence to the PTW
system is necessary.
The site was kept clean properly. All rubbish is put into the waste bags that have been set up around the
site. The materials (insulations, scaffoldings, and tool boxes) were appropriately positioned, and the area
was well barricaded.

Conducted safety moment –Moving vehicles at bouroge retubing site. The topics covered are as follows
Stay Alert : Always be aware of your surroundings at all times and pay attention to any moving
vehicles nearby.
Wear High-Visibility Clothing: Wear bright, reflective clothing to make yourself more visible to
vehicle operators.
Follow Traffic Laws: Follow all traffic laws and signs, and never assume that a vehicle operator
sees you.
Keep a Safe Distance: When walking near moving vehicles keep a safe distance and avoid
standing in blind spots.
Use Proper Equipment: Use caution signs, traffic cones, and other safety equipment as
necessary to alert vehicle operators to your presence.
Report Hazardous Conditions: Report any hazardous road conditions or vehicle malfunctions
immediately.
4. Level 2 HSE walkthrough –Borouge Retubing (pm, Rajendrakumar, Ansil, Naseer) 14/02/2023
With our project manager and Borouge HSE engineer, a walkthrough of borouge retubing was
conducted. Everything was found to be in order during our HSE walkthrough. The observations made
during the walkthrough are listed below.
Positive Observations
1. The area where lifting activities were taking place was properly barricaded, signage was provided, and
lifting activities were supervised by riggers. On-site, an approved lifting plan was available.
2. All work was done in accordance with a permit system, and permits were readily available on site.
4. The entry guard (4111) for confined space is well-versed in emergency response protocols, as well as
where manual call stations and phone booths are located.
Negative Observations
3. Damaged cutting discs and equipment were found near the store's entrance.

HSE Engineer from borouge asked our confine space attendant (4111) several questions during our
walkthrough. He answered them all clearly and had good understanding of the emergency response
protocols, the locations of manual call points, assembly points and telephone call booths.
It was found during our walkthrough that one of the scaffolders (ALTRAD) was utilizing a safety harness
that was not color-coded.
Ensure that all tools and equipment are inspected and color-coded.
All safety harness were inspected and color-coded. For all involved employees, a special toolbox
meeting was conducted.
(e) Inside the store, wooden hammers are placed - Borouge Retubing
Wooden hammers are kept within the site store, as was found during our inspection.
Damaged cutting discs and equipment were found close to the store's entrance during our HSE
walkthrough.
Inside the store, proper housekeeping should be done. All unwanted materials should be removed.
The lack of a clear separation between one of our riggers and the suspended load was found. He was
remaining quite close to the suspended load as he was using the tagline.
Tagline when used while lifting, a clear space should be kept between the lifter and the suspended
weight so that the load can be controlled and line rotation is avoided. Training should be given to all
riggers
Immediately stoped the activity. Conducted special TBT for all riggers.
