 Land Disturbance Permit.
 Approved Method Statement.
 Latest revision Issued for Construction drawing/s.
 Safe Access Handover Certificate received from KCSA Construction.
 Risk Assessment approved by the KCSA SHE Manager.
 All required QMS documentation in place and approved by KCSA QMS Manager. (Approved QCP, IFC drawings etc)
 Setting out has been checked and surveyed by KCSA Surveyor.
 Buried Services scan report from Survey dept. attached. If not state your reason in comments below. YES NO
 Are there any known future structures to be constructed in this area? YES NO
 If yes is ticked above, ensure future structures are indicated on the block plan.
 KCSA Construction Project Manager Infrastructure to confirm.
Ensure all the above supporting documentation are attached hereto prior to signing.
