Re: Dissemination of Ready-to-Print Layout of the Checklist for Documentary

Requirements

Sir/ Ma’am,

Respectfully transmitting the ready-to-print layout for the prescribed Checklist for Documentary
Requirements, for your perusal. In response to the requests of liaisons for a template to ease
reproduction, the document is formatted to fit in a long bond paper (8.5” X 13”), portrait
orientation, to be attached in the voucher in lieu of the summary list.

Likewise, we wish to take this opportunity to remind the mandatory compliance to the prescribed
checklist starting February 20, 2023, pursuant to Memorandum Order No. 307-2023 issued by
the Office of the City Mayor. Thus, the Office of the City Accountant will no longer entertain
vouchers submitted on such date without the prescribed checklist attached.
