PARENTAL CONSENT

I confirm that I, _______________________________________, the parent/legal

guardian of _______________________________________, consent and permit my child to
join their group experiment and data-gathering procedures in connection with their research
study. I understand that my son/daughter’s participation will be of great help in the conduct of
their research project.

I hereby allow him/her to participate in the following activities outside the school premises:
· Meeting with Mr. Duovani Regondola at Camarines Norte National High
School.
· Collecting food waste in Daet Public Market.
· Going to Daet Municipal Office and the provincial capitol to obtain a
financial grant.
· Visiting College of Agriculture and Natural Resources (CANR), CNSC Labo
Campus, to inquire about feed formulation and to acquire information from
past research about BSF in the institution’s library.
· Collecting pili pulp from the Seed Farm in Calasgasan, Daet, Camarines
Norte.
· Using the pili presser available in Capacuan, Paracale, Camarines Norte.
· Collecting malunggay in Tagongtong, Mercedes, Camarines Norte.
· Delivering the raw materials to Malasugi, Labo, Camarines Norte for drying.
· Inspecting the progress of the dehydration of the raw materials in Malasugi,
Labo, Camarines Norte.
· Taking the treatment samples to a courier service that will deliver it to DOST
Pampanga.
· Canvassing materials for the chicken coop.
· Assessing the area in Paracale, Camarines Norte where the chicken coop will
be constructed.
