Directions: Each student is to be rated on performance in the return demonstration. Careful evaluation to be taken
to compare the jobs and tasks actually performed by the student nurse with those expected from the full
performance of that assignment. Keeping the guidelines in mind, please complete the Performance Evaluation
Tool. Then determine an overall rating, giving weight to various performance categories as appropriate to the
particular assignment. Please indicate remarks in the space provided if needed. Overall rating must be considered
with the following standard definition.

UNOCCUPIED CLOSED BED 5 4 3 2 1

1 Performs handwashing/hand hygiene. Dons gloves

2 Assembles all materials and place on the bedside chair and must be
stacked according to the order of use.

3 Places the folded bottom sheet with its center fold on the center of the
bed. Make sure the sheet is hem side down.

4 Unfolds the bottom sheet on bed with smooth side up and tucks at the
head part, makes a miter corner and tucks from head to foot part of one
side.

5 Places waterproof underpad at the center of the bed about 18 inches


from the head part, covers and tucks along the side.

7 Unfolds top sheet and tuck at the foot part of mattress hem side up and
make a modified mitered corner at the foot of the bed. Does not tuck
along the sides.
8 Places the upper hem about 16 inches from the head part of the mattress.

9 Lets the top sheet hang on the side.

1 Moves to the other side and secure the bottom linens.

1 Tucks in the bottom sheet under the head of the mattress, pulls the
1 sheet firmly, and miters the corner of the sheet.

1 Goes on the opposite side of the bed and draws on the pillowcase. Hold
2 the pillowcase with a firm grip in one hand. With the other hand, open the
pillowcase from the open end and fold the back over the closed end
(inside out).

1 Pick up the pillow with the hand holding the pillow case. Invert the pillow
4 for the pillowcase to drape over the pillow. Pull the pillowcase ove the
pillow with the other hand

1 Fluffs the pillow fitting the corners well into the case. Folds the excess
5 pillow case on the side.

1 Places pillow at the head part of the bed with opening away from
6 the entrance.

1 Hang gown, towel and face towel at the head board.

1 Performs steps 1 and 2 of closed unoccupied bed.

1 Unfolds bottom sheet as in close/ unoccupied bed.

1 Proceeds with the top sheet as in close bed, only fold back top sheet
8 and fanfolds towards foot part or make a side flap.

1 Fluffs the pillow and place at the head part

2 Hangs gown, towel and face towel at the head board.
