NPP0085-LTR-JEC-NPP-0381. Dated 04 December 2017, regarding the above subject. HPP has reviewed the submitted documents and would like to advise the contractor that the submitted documents cannot be reviewed and approved at this stage due to the following: 1. This method statement, ITP and Risk Assessment should be reviewed based on Pile Design and Specification which are not developed by the Design Consultant. 2. Item 1.2: List of applicable documents are incomplete. Specification references and Building code references are missing. 3. Item 1.3: Attach sample copy of hot and cold work permit. Refer/mention all statutory permit requirements to perform piling operation so that all the personnel involved in this operation are aware of the requirements and compliance accordingly. 4. Item 2.2: Roles and Responsibilities. Roles and responsibilities of piling Sub-Contractor personnel are very brief and incomplete. Please elaborate in consultation with the Piling Sub-Contractor. Item 4.1: Segregate and list the Main contractor and Piling Sub Contractor manpower 6. Item 4.2: Segregate the equipment arranged /supplied by Main contractor and the Piling Sub Contractor 7. Ttem 4.3: Add the following in the list + Approved concrete spacer + Tremie pipe 8. Item 5.1: Health and Safety + The HSE provisions are general. Please be specific to Piling Work activities. + Permits and Certificates: Please add Drilling Rig in the list and attach checklist of the plant before starting the job. Item 5.1.1: Emergency Response: Please rewrite with reference to the Piling Work activities Item 5.2: Environmental Issues: This item to be addressed by the Piling Sub-Contractor especially related to substantial quantity of mud, slurry and excavated material. Excavated material and water may contain toxic gases i.e. Hydrogen sulfide etc. Item 5.2.1: Aspect Identification and Mitigation Measures: Please see comments item 5.2 above and modify the table accordingly. Item 6.1: Sequence of Work: The statements are general. Please elaborate the sequence of work with reference to the piling activities for oil Tanks and Silos et, Item 6.2(A): Ground Investigation Report: The statements are general. Before starting the work, the Piling Sub Contractor's Engineer and field staff must be aware of the ground condition as per the Geotechnical report. Item 6.2(B): Unexpected Ground Condition: Under such a situation the Contractor shall find the solution in consultation with the Design Consultant, Geotechnical Specialist, and Piling Sub contractor. Item 6.4: Installation tolerances (Setting out, Position, level, Verticality etc. shall be as per the Piling specification and so far it has not been produced yet. Item 6.4(F): Forcible correction to pile: Forcible correction to pile shall not be allowed until and unless itis studied , verified and allowed by the Design Consultant and Piling Sub contractor. Item 6.5(C): Damage to the Pile: Proposed sequence and timing for driving or boring piles to be the part of this method statement, Item 6.6(B): Steel Casing Removal: Stee! casing to be removed before Initial seting time of the Cement. Item 6.7(A): Please add the following in 3rd bullet point: Inno case, the Drilling will be continued below the steel Casing level except Stee! casing reached the rock level. tem 6.10(E) Placing of Concrete: sub item (g): Please elaborate the method of concrete vibration as proposed in this section. Item 6.11: Testing (A): Concrete Sampling and Testing: Pile and Pile concrete testing plan to be as per the Pile specification and Concrete specification. So far these are not yet produced Japproved. Item 6.11: Testing (B) sub item (2a): Type of testing on pile and frequency of testing to be as per the approved Pile specification. This document has not been produced yet. Inspection Test Plan: Activity nos. 2.1, 2.2 & 4.1 to be the Hold Point for PMC.
