This EXTRAJUDICIAL SETTLEMENT OF THE HEIRS OF VIRGILIO V BAUTISTA

Christine Village Brgy. Biga, Tanza, Cavite;

DANIEL C. BAUTISTA, of legal age, Filipino, Single, residing at 198 Camias, Magalang,
Pampanga;

VIRGILIO C. BAUTISTA JR., of legal age, Filipino, Single, residing at Blk J Lot 25 Christine
Village Brgy. Biga, Tanza, Cavite

DONNA MAY B. TAN, of legal age, Filipino, Married, residing at 1845 Nazareth St. Tabon
Road, Malaria, Caloocan City;

SHERLYN VON C. BAUTISTA, of legal age, Filipino, Single, residing at Punta 1, Tanza,
Cavite;

WITNESSETH, that:
WHEREAS, we are the sole and legitimate heirs of the deceased VIRGILIO V BAUTISTA
who died on 26 July 2019 at Christine Village-Biga, Tanza, Cavite; copy of his death certificate is
hereto attached;

WHEREAS, VIRGILIO V BAUTISTA intestate, without will or testament, and without any
outstanding debts in favor of any person or entity;

WHEREAS, VIRGILIO V BAUTISTA is a member/depositor of ACDI MPC, ACDI TRECE

MARTIRES BRANCH with a deposit in the total amount of Fifty Thousand Pesos Only
(P50,000.00)

Now therefore, for and in consideration of the foregoing premises, and being with full
capacity to contract, and of our own volition hereby settle and adjudicate among ourselves the
above sum of money, in equal shares pro indiviso as follows:

MA. SUSANA C. BAUTISTA P30,000.00

DANIEL C. BAUTISTA 5,000.00
VIRGILIO C. BAUTISTA JR 5,000.00
DONNA MAY B. TAN 5,000.00
SHERLYN VON C. BAUTISTA 5,000.00
TOTAL P50,000.00

That the HEIRS hereby warrant that all taxes and obligations necessary or required has
been paid and that hereby hold ACDI MPC free and harmless from any and all claims arising from
the distribution and/or release of the monetary claims/savings deposit of VIRGILIO V BAUTISTA.

That the HEIRS hereby covenant and further warrant that should there be preterition or
omission of other compulsory heirs in the direct line, whether living or not at the time of the
execution of this Deed, will not invalidate/nullify the terms and conditions of this Deed. Instead, the
HEIRS shall proportionately oblige themselves to pay to the omitted Heirs or the latters’ heir(s) the
share which belongs to him/them, in accordance with the rules of succession under the Civil Code
of the Philippines.
