PRELIMINARY ACTIVITIES

The Class Adviser calls _____________________ for the Opening Prayer and a
warm welcome was given to the parents by the class adviser.

CALL TO ORDER

After, checking with the attendance sheets with the number of present against
the number of students, the Class Adviser declares a quorum and starts the
meeting at _____________.

PRESENTATION OF THE AGENDA

The Class Adviser presents the school agenda, the adviser’s agenda and asks
the parents whether they have other agenda they wish to be included in the
HPTA Meeting.

Agenda:

2. Intensified wearing of face masks
3. Wearing of school ID
4. Authorized Fees
5. Other matters

Conference Proper:

The Class Adviser welcomes the parents for the 5th HR-PTA meeting then
discussed the agenda.

1. Students’ Academic Performance

The class adviser reminded the DepEd Order No. 8 2015 (POLICY
GUIDELINES ON CLASSROOM ASSESSMENT FOR K TO 12 BASIC
EDUCATION PROGRAM). It stated that when a learner’s raw scores are

consistently below expectations in Written Work and Performance Tasks, the

learner’s parents or guardians must be informed not later than the fifth week of
that quarter. This will enable them to help and guide their child to improve and
prepare for the Quarterly Assessment. A learner who receives a grade below 75
in any subject in a quarter must be given intervention through remediation and
extra lessons from the teacher/s of the subject.
The class adviser explained why there are students who failed in a
certain subject. The following are the components in every subject.

2. Intensified wearing of face masks

The school reiterates the wearing of face masks on campus due to the
rise in COVID 19 cases in the city. The class adviser emphasized to the parents
the importance of using face masks to prevent the spread of the virus.

3. Wearing of School ID

The class adviser reminded the parents that their children should arrive
at school with ID for identification and security purposes.
