Greetings of peace!

In accordance with Division Memorandum No. 0289, s. 2022 released last May 6, 2022, I would
like to request from your good office that Cayacay High School will be allowed to conduct a
Limited Face-To-Face Graduation and Moving Up Ceremonies for School Year 2021-2022. The
school has been issued with Safety Seal by the Regional Director last March 22, 2022.

The learners, parents, teaching and non-teaching personnel and the rest of the stakeholders have
been longing for the conduct of physical commencement rites. It has been two (2) School Years
that candidates for graduation and completion together with their respective parents and
guardians had to satisfy themselves with virtual platform.

Attached are the authority issued by the Local Government Unit of Alicia and resolution of the
school’s Parent Teacher Association (PTA).

RESOLUTION NO. 1
Series of 2022

“A RESOLUTION SUPPORTING LA HACIENDA NATIONAL HIGH SCHOOL ON THE IMPLEMENTATION OF

EXPANDED LIMITED FACE-TO-FACE CLASSES FOR GRADES 8, 11 AND 12 IN THE FOURTH QUARTER OF
SCHOOL YEAR 2021-2022.
WHEREAS, the Department of Education (DepEd) – Bohol enjoins all schools, public or private, in the
Division to implement the Expanded Limited Face-To-Face Classes in the Fourth Quarter of the School
Year 2021 - 2022;
WHEREAS, the parents and community are supportive in the conduct of limited face-to-face classes
instruction;
WHEREAS, the willingness of the parents is due to the reason that monitoring of learners and parents is
a problem because of inaccessibility of internet connection and cellular signal and restrictions

WHEREAS, this resolution is one of the requirements set by the Department of Education that need to
be complied by the schools;
WHEREAS, the Parents Teachers Association (PTA) is willing to support and monitor the process of the
implementation;
NOW THEREFORE, on motion of Mrs. Vangelita Madera duly seconded by Mr. Roberto Tuyor, be it:
RESOLVED, as it is hereby RESOLVED, by this august body, to support Cayacay High School for the
implementation of the Expanded Face-To-Face Classes for Grades 12 in the Fourth Quarter of School
Year 2021 – 2022;
RESOLVED FURTHER to furnish copy of this resolution to the Office of the School In-charge of Cayacay
National High School and all other offices concerned for their information, reference and
appropriated .action.
“UNANIMOUSLY APPROVED.”
