A. LEADERSHIP, POTENTIAL AND ACCOMPLISHMENTS
a. Introduced any of the following which has been adopted or used by the school
or district
1. A Project Proponent and Team Leader of Project SmSMILE ( Speed in
Multiplication ) from 2019 up to present. As a the team leader of the said project, I am
one of who conduct the project from the narrative, project proposal and matrix as well
as the activities to be given to the learners.
2. A Project Team Leader for the Project Paksa ( Pagyamanin ang Kaalaman sa Araling
Panlipunan). In 2017 the said project was a School to School Partnership Project which
was continue of the school after a year. In 2019, the project was continue to implement
in the school because it found out that it was very effective to raise the academic
performance of the learners.
b. Served as Subject Coordinator or Grade level chairman for atleast one (1) year,
or as adviser of school publication and not considered part of the regular
teaching load.
1. A grade level chairman for Grade 6 for 9 years.
2. A school paper adviser for 9 years, a coach and trainor of Young Campus Journalists
and the who makes the school paper.
3. A DRRM Coordinator for 10 years preparing the accomplishment reports in DRRM as
well as attending semionars and trainings.
4. An Adopt-a-school coordinator, being coordinator one of my tasks is to prepare the
summary of donations received by the school.

c. Serve as Chairman of a Special Committee such as Curriculum Study
Committee to prepare Instructional Materials, Committee to prepare School
Programs and Discharge the work efficiently.
1. I served as Chairman in the 2018 School Press Conference. Its main goal was to
develop the learners’s skills in writing and to select the learners who have a potential to
be the contestant for the upcoming District Press Conference. Being the chairman of the
said activity, I was the one who dessiminate the tasks to my co-teachers and prepared
the matrix and action plan for the activity.
d. Initiated or headed an educational research duly approved by educational
authorieties.
1. I have an Action Research entilted. Project E VoW ( Enhance Vocabulary Words )
and Its Effect to the Academic Performance in English of Grade 6 Learners at Oogong
Elementary School, District of Santa Cruz, Division of Laguna. As a researcher, I made,
the pre-test and post test, where I adminstered it during the Second and Third Quarter. I
also analyzed the result of my action research where the result was already interepreted
with finding, conclusion and reccomenedations.
e. Coordinator of Community Project on activity or of a program of another
agency or coordinator of a rural service improvement activity in a community
such as feeding, nutrition, agro-industrial fair, etc. for the last two (2) years
1. I served as Chairman in Feeding Program in our Community Service on September 1
2018 held at Provincial Jail Brgy. Poblacion Santa Cruz, Laguna.
2. On the following year , February 8, I served again as Chairman in Feeding Program
held at River Side, Sitio 5, Brgy. Oogong Santa Cruz, Laguna.

Pedro City, Laguna on August 15-17, 2018
3. 14th Place bilang tagapagsanay sa Paligsahan sa SULKAS TULA, na ginanap na
Pampurok na tagisan ng Talinosa Filipino 2019 sa Paaralang Elementarya ng Silangan
Sta.. Cruz Laguna.
4. 5th Place bilang tagapagsanay sa Paligsahan sa MADULANG SABAYANG
BIGSAYWIT, sa ginanap sa Pampurok na Paligsahan sa Bigsaywit 2018 na ginanap sa

8. In 2021-2022, I was nominated as an Outstanding Teacher in Oogong Elementary
School and represented the school in the District GAWAD HUWARAN for Outstanding
Teacher last December 2022.
B. DEMONSTRATION TEACHING
1. I rendered as Demonstration Teacher during the 2019 DISTRICT DEMO-TEACHER
TEACHING FESTIVAL held on February 8, 2019 at Oogong Elementary School. The
subject I discussed was Araling Panlipunan and the topic was Kilusang Propaganda.
