III. PRESENTING PROBLEM

Johaira, a walk-in client came to the office of municipal social welfare and
development office to avail of a solo parent ID.

IV. History of the Problem

On May 29, 2022, when Johaira Find out that her husband started cheating on
her, not just once, but twice until she lost counting, she’s aware that her husband was
an alcoholic before they got married, he drink a moderation time, but when he got fired
to his job he became a drunkard. It starting from that day he cheated on her with
different women and not be able to provide what they needs especially when it comes to
her children. She decided to separate from him due to an unhealthy environment for
herself and for her children and provide a secure environment in order for them to grow
as good children or people someday.
V. Background

The client

Johaira, the client, 36 years of age, was born on December 12, 1987, at Purok
Malinawon, Barangay Poblacion Polomolok South Cotabato, she was blessed with two
children, her daughter name Jonaisa and her son named Johari. Her daughter currently
studying at Polomolok Central Elementary School while the youngest is currently
staying at home and being taken care of by her cousin.

At present, Johaira is working at WWA Enterprises for almost two years.

Currently, they live at Purok Malinawon, Polomolok South Cotabato, aiming to gain
support by the Local Government Unit LGU to provide for their basic needs as well as
the education of her children even in the small amount, for their daily lives, her salary as
a sales lady in WWA enterprise is not enough for them to live and spend to their basic
needs and other expenses.

The Family

Johaira is 36 years old, born on December 12, 1987, and raised in Purok
Malinawon, Poblacion Polomolok South Cotabato. She is the youngest among the three
siblings, namely Masdi and Norhaima. Her father’s name is Sirad while her mother is
named Nanang. Johaira started her primary education at Polomolok Central Elementary
School, even though she experienced early difficulties in her life, and as a student, she
was able to finish grade school.

She took her secondary at Poblacion Polomolok National High School, the
difficulties that she experienced became worst, thinking that she should stop studying
for not have financial support from her family, she graduated high school in the year of
2003-2004. She tries and still pursues her college but not be able to finish it, she just
steps up in the first year of college and the first semester of second-year college. She
stopped studying because her mother is a complete housewife and her father doesn’t
have enough source of money labor to provide ide her education expenses.
 On the day that she stopped studying, she immediately find a job, thinking to
extend help to her family with financial support, her other siblings got married and her
eldest brother lived with them while her sister built her own family and live in Barangay
Magsaysay. She work as a sale clerk in Gaisano Polomolok at the age of 19, at that
moment she ease the burden of her family that they are carrying.

And so now that she became a solo parent and aims to get help from Local
Government Unit and avail of solo parent ID and benefits just as educational benefits,
medical assistance, and more. It is a huge help if there is more additional support and
assistance for having a solo parent ID.

The Community

Johaira has a good community. She was surrounded by kind people. She has a
pleasant relationship with her neighborhood and was reared in a good community.

VI. Assessment Statement

Johaira has been a solo parent since her husband left her, accordingly, she is
having a hard time providing for her two children. Being a solo parent give her difficulty
wherein she couldn’t provide enough support to her children’s needs.

The client is a hardworking and multitasking mother, she is passionate about her
job and determined to be a better mother and employee for her children. She is doing
her best to provide for the needs of her children. She is not just working as a hard
mother but also a mentally stable woman with not show her weaknesses in front of her
children. Johaira wants to become a role model for her children.

If the client won’t be able to be given the privileged to avail the Solo Parent ID
she could not gain the benefits of it, there is a huge tendency that she could not avail
other assistance and support from the Local Government Unit (LGU). Furthermore, if
the client will be able to avail the solo parent I.D., it will surely be a huge help for her to
provide for the daily needs of her children.
VII. Action taken

Goal: To facilitate the client, Johaira ask for assistance and be able to avail the
solo parent ID on May 23, 2023-June 05, 2023.

VIII. Recommendation

After all the data gathering and thorough assessment of the case, the Social
work student humbly recommends the clients avail of solo parent ID.
IX. Evaluation and Termination

The first time I saw my client, she looks problematic, like not be able to focus on
her children, and her appearance emphasized that she was not well, her figure
screamed distress about something, after that, I interviewed her about her basic
information and asked why she went to our office, and due to that, I identified what are
her problems and what her intentions. Since that day, the counseling sessions help her
to overcome slightly of her problems and finally, compared to before, she looks
genuinely happy and contented with her life together with her children.

In the case of Johaira, the client is qualified to avail of the Solo Parent ID. She
complied with all the needed requirements and submitted it to the Municipal Social
Welfare Development and Office, Polomolok South Cotabato on June 05, 2023.
Moreover, the goals and objectives are met therefore the case is terminated.
