II. PROBLEM PRESENTED


Welson P. Mantos doesn't have sufficient and permanent income. Hence, he
needs regular financial support to prevent him from hunger and provides his basic
needs.

III. HISTORICAL BACKGROUND


A. CLIENT AND FAMILY
Welson Mantos grow up with his oriented family. At his young age, his
performance in school was good, and he graduated high school. Unfortunately, he didn't
continued to study in college because of the crisis financial situation on his family. So
that Welson Mantos find a job in Manila, until he got married early and he had three
child. His past is just so bad, because Welson Mantos and his wife marriage is not
perfect 'til their marriage and family were ruined. Then Welson Mantos, decided to go
back at mindanao with his three children to start over.
Further, Welson Mantos and his children got separated when they returned in
mindanao, just because his eldest child got scholarship program for disabled person in
Zamboanga City, then his elder child worked at TESDA in Pagadian City and also his
youngest child graduated high school last year with the help of taking care of his ex-
wife, while he guarded the farm where he work, but so far Welson Mantos and his ex-
wife has no communication even when their youngest child currently residing with his
elder child, while studying in college.

B. SOCIAL CONDITION
Welson Mantos living alone in a simple little house that is close enough for him to
relax, rest, sleep, cook, or else. Because he is just living alone at his place where he
work.

C. HEALTH CONDITION
At the moment Welson's health is still strong and his muscles, nerves are
functionally working, but he is moving slowly and he can hardly to hear if someone talk
to him, so they should be talk him loud and clear to avoid conflicts.

D. ECONOMIC CONDITION
Welson Mantos is a 60 years old that needs government financial assistance
because he was only a tenant . His income per month is insufficient to support his needs
and it isn't permanent. He doesn't have any income aside from the share of coconut
croppings every after 3 months and also he doesn't have own land and any insurance.
He has no regular or permanent financial support from his elder child because the salary
of his elder child is only 648 php per day.

IV. ASSESSMENT STATEMENT


Client was only a tenant of a secluded farm. His income is insufficient to support
his basic needs. He doesn't have any income aside from one share of the coconut
croppings. He doesn't have of any kind from government beneficiary. Because client
belongs from a poor family. He didn't finish his study due to financial crisis, that is why
his parents hadn't financially support on his study during his college.
Despite Welson Mantos economic struggles and insufficient income even in his
poverty, he also found a small way to prevent illness and hunger. Now, that he is older,
there is a possibility that he may feel some minor ailments. So he needs financial and
medicines help support in case of whatever emergency happen. Then so the financial
regular support from the government is the immediate needs that need to be address,
like social pension program and senior citizen's welfare for eligible indigent senior
citizen and also receive seedlings or any root crops, because it is the center of his
livelihood and this is what he can do. So that, he can lessen his poverty and avoiding the
incidence of hunger.

V. EVALUATION
Implementation of social pension for the client is on process. Then so, the social
workers continue and processing on it. And the social work student checking update to
monitor the updated beneficiary social pensioner senior citizen list and to ensure the
client status in Social Pension Program.

VI. RECOMMENDATION
While follow up and monitoring the social pension for the client, as well as social
work student encourage the client to have medical check-up of his hearing impairment
for attachment to enroll the PWD organization. Then assist him to comply his
requirements to apply for PWD organization. After that, case shall be turn it over to
PWD staff for continuous processes.

The Social Work Student conducted an interview and had collects all the necessary
information about the client and build a relationship the client. For reasons, to engage the
client in dealing effectively with the problem , and giving the client opportunity to have fully
explain the facts , as well as their interests in life to develop or improve.

Social work student have observed , this conducted interview of the client, was good and
it feels comfortable to talk with him. Because the client was very formally answered my
questions.

Moreover, social work student had meet the clients determination in resolving and
improving his quality of life. The focus of this client intake , which is the process of acquiring the
personal information that can identify and relate to a living person and get the client's historical
background to have analyze and formulate the possible problem recognize from the client's
information.

However, some of the common information from the client person's life will use into my
assessment statement to meet the client needs and to be follow my intervening plan.

The array of development intervention for client is done to assist client to improve his
living condition. Identifying internal resources in client's information and the assessment
statements were made to fast track this plans if improving and development.

The Senior Citizen Affairs Officers under MSWDO was instrumental in synchronizing
development interventions and support for the eligible indigent senior citizen. Referral
mechanism to member agencies and existing resources which can provide privileges, related
financial support and legal support were made by the personnel who anchored the
implementations programs for Senior Citizen's Welfare and Development.

In the future, the MSWDO in charge in Senior Citizen's Welfares, will also tap my client to
the other organization, as social work student recommend my client to have a medical check-
up of his hearing impairment for attachment to process his enrollment in PWD organization.

of 04:00 pm on April 29, 2022. The worker had her intake interview on April 29, 2022.

Together with client, visited the OSCA office on May 02,2022. Seek assistance on the
requirements.
The social workers had released the Client’s OSCA ID and Purchase Booklets for Medicine
on May 02, 2022 at the office of the Senior Citizen Affairs.
