AFFIDAVIT OF NON- COHABITATION

I, SIGAY M. LAUNTE, of legal age, married and a resident of Purok

6A, Tambacan, Iligan City, after having been sworn to in accordance with
law hereby depose and states that:

1. I am currently married to Naga M. Launte;

2. However, the above stated person and I have been separated for
almost 18 years due to his act of abandonment against me and my
children;

3. Both of us have no plans to reconcile with each other now or in the

near future and that the custody and sole parental care of my children
is currently under me;

4. I am executing this affidavit to attest the veracity and truthfulness of

the foregoing statements and for the application of my Solo Parent ID.
