ROLES AND RESPONSIBILITIES

➢ PTSC OFFSHORE 1 will invite the Master of AHT onboard for pre-job meeting and discussion
for this anchor handling job. Discussion shall include risk assessment which includes JSA/JHA
and mooring Procedure.
➢ Master of PTSC OFFSHORE 1 will be in command during the operation. To apply PTW before
the operation and to manage the operation to be carry out in a safe manner. Manage the crew
duties accordingly and in charge of manoeuvring the vessel.
➢ Chief Engineer will be standby on winch control. Make sure all machineries in good working
condition. To ensure that main propulsion including bow thruster, pumps and hydraulic
winches are all in good condition
➢ Bosun will be standby on main deck. In charge of deck crew and make sure all anchor handling
equipment are in good working condition. Make sure the safety of other deck crew during the
operation.
➢ Crane operator will be standby in the crane house. To check and test the crane performance
and make sure in good working condition.
➢ Safety officer will be standby on main deck, and control during lifting / transfer of materials.
He will conduct toolbox meeting and explain the JHA and procedure before commence the
operation. Keep monitoring the weather condition and other vessel in vicinity for smooth
operation. Make sure all the crews involve wearing full PPE and following the correct
procedure.
➢ Radio operator will be standby in the bridge / Radio room, monitoring all activity and record
into the logbook.
➢ Anchor handling Master has full command on the AHT side, communication to establish
between AHT and AWB prior starting any work or activity.

APPROACH TO PLATFORM AND WORK AREA

➢ PTSC OFFSHORE 1 will NOT commence mooring operations from the platform until the
following conditions are satisfied.
➢ Communication channel via VHF is established between the Master of PTSC OFFSHORE 1, AHT
and Platform Controlling Authority of Restricted Area.
➢ Entry permission must be granted by the controlling Authority before vessel is to enter or
move within the said 500 m restricted area.
➢ Sea condition, visibility and weather conditions are suitable for Anchor handling operations –
a limit 2 m sea and swell height, while wind of 25 knots or Beaufort scale force 5.
➢ The weather forecast and visual meteorological observations are good enough to complete
the anchor handling and mooring operations.
➢ All mooring operations should as far as possible be conducted during daylight hours.
➢ The prevailing direction and rate of the current at site are known and are within limits with
respect to the platform and underwater structures including pipelines.
➢ Surveyors / operator should be on board during operations to operate the BTM / TMS /BMS
system, which has been tested and full operational.
➢ All navigation equipment, anchor winches and propulsion system (AHT) are to be tested and
confirmed to be in satisfactory working order.
➢ A toolbox meeting involving the workboat Master, AHT, CSR and platform authority is to be
arranged to discuss the mooring procedure prior commencement of the operation. This
discussion may be conducted via VHF if it is impracticable to meet on board.
➢ The approved permit to work and risk assessment should be completed prior to the job (see
SMS FORM 111-Risk Assessment Form) which should have included the JSA/JHA as part of
the discussion with all relevant parties.

Hazards That MUST Be Addressed in the HIRARC but not limited to the following :-

➢ Work wire entanglement – Controls ; good anchor handling > practices Work wire
entanglement – Controls ; good anchor handling practices Crossing near pipelines – Controls
> steer/manoeuvre vessel steadily.
➢ Work wire under tension – Controls ;stay clear form wire under tension.
➢ Deck flooding – Controls ;work in buddy system with full PPE.
➢ Search and sweeping area approach by other vessel/passing by vessel – Controls ; effective
look out and establish radio communication.

PRECAUTIONARY MEASURES WHEN DEPLOYING

1. Wind speed at commencement not to exceed 25 knots.


2. Wave height not to exceed 2 metres.
3. Visibility on approach and fixed platform must be good.
4. No any observed indication of squall occurring.
5. All anchor to be decked on workboat and AHT, safely secured with no possibility of dropping
into the sea.
6. There should be no transfer of anchor between vessels over pipelines.
7. Anchors should always be decked when crossing over pipelines.
8. Sufficient tension is to be maintained on all anchor during deployment in order to keep clear
of any pipe lines.

OPERATIONAL CONDITIONS

1. Anchoring Restrictions

➢ Anchor restrictions shall apply when mooring near existing structures or above existing
pipelines/and or subsea facilities. In the event an anchor line is to be crossed an existing
pipelines/and or subsea facilities, a spring buoy shall be installed on the anchor line vertically
above the crossing, in order to maintain a vertical distance as per Table 2 above the highest
point of the existing pipelines/and or subsea facilities. The spring buoys available with the
AWB has a net buoyancy of 5 metric tons, which will maintain the clearance even if the anchor
line is slacked.
➢ Horizontal clearance between any anchor and an existing pipeline/and or subsea facility
should be not less than 100 m. Where the drag path of the anchor is towards the asset, drag
embedment anchors should be located more than 300m radius from the point where the
anchor line crosses the pipeline, cable or subsea structure.
➢ If an anchor is to be moved over or within 50m of any subsea facilities, the anchor shall be
decked on the AHT and double secured anchor line in shark jaw.
➢ In Case Where anchor line passing through 50 meters zone existing structure (jacket tanker)
is unavoidable due to operational requirements, manoeuvre will be performed as per
approved anchor mooring pattern and a dedicated assessment of environmental condition;
vessel motion and behaviour will be done. Vessel movement speeds in such areas will not
exceed 0.5 knots (0.25 m/s).
➢ Where AWB is unable to comply with the above requirements, AWB shall submit their
alternative anchor pattern to COMPANY for review and approval.

2. Installation Tolerances - Anchor will be positioned within a 25m radius of the intended position
prior to tensioning. After tensioning, the cable bearing will be within +/-2 degree of the intended
bearing.

3. Weather Forecast - During the anchor handling period, the weather forecast will be regularly
followed by the weather reports received on board. The weather evaluation will be done by the
Master, in agreement with the AHT Master and Barge Master, to decide over the safety of the anchor
handling activities.

4. Operational Weather Window - A favourable weather window of at least 48 hours should be


available to start anchor handling operations.

5. Limiting Weather Criteria - The following limiting criteria shall be followed:


a) Wave height: 2.5 meters
b) Wind speed: 25 knots

6. Working Condition:
I. Anchor handling work shall be completed or carried out in daylight.
II. In the event where the tentative plan for anchor handling work cannot be completed in
daylight and require continuing during night hours, authorization to proceed with the work
MUST be obtained or granted from the PCSB Marine Controller, AWB Master and CSR on
board.
III. All offshore operation at field shall follow as per APPENDIX 2 - PCSB Manual of Permitted
Operation (MOPO).

7. Anchor Running at Night and Conditions of Poor Visibility - Normal installation operations
necessitate working 24 hours when running anchors in situations of poor visibility e.g. in hours of
darkness and hazy conditions, extreme precautions shall be undertaken to ensure a safe operation
and the following additional requirements shall apply:
I. Besides using the radar of the AHT, a look out shall be assigned to spot any obstructions during
running of anchors at night.
II. AHTS shall operate at reduced speeds and searchlights switched on to provide better
illumination in the vicinity of offshore installations and platform complexes. These precautions
shall be taken in addition to the BMS, which shall always be used.
III. Anchor buoys used are large enough to act as radar reflections.
IV. The anchor location is regularly monitored in reference to the anchor buoys on water surface.
V. AHTs shall operate with searchlights switched on to provide better illumination in the vicinity
of offshore installation and platform complexes

8. Simultaneous Operations (SIMOPS)


I. Where two independent activities may impact each other, for example due to proximity of
construction facilities, then SIMOPS exist. As a result, written guidelines and clarity shall be
required to define hierarchy of roles, responsibilities and allowable simultaneous activities.
II. As a general philosophy, SIMOPS shall be avoided or minimized wherever possible.
III. However, during the offshore installation work phase, it is possible that the COMPANY Drilling
Rig and/or Supply Vessel will be in field. As a result, management of SIMOPS will be required
during the operations.
IV. No anchor will be dropped inside the anchor pattern of any other work vessel operating in the
field vicinity without approval from relevant parties

ANCHOR HANDLING PROCEDURE

General Anchor Pattern - For all the pre-installation preparation, specific anchor pattern drawings will
be developed. Each anchor position is studied accounting for the most appropriate AWB heading,
position for the pre-installation activity and accountable parameter.

Pre-Anchor Job Coordination Meeting - Prior to anchor handling operation, PETRONAS Marine
Controller shall conduct a Pre-Anchor Job Coordination Meeting. AHTS Master shall attend the Pre-
Anchor Job Coordination Meeting with the AWB Master to discuss the anchor handling procedures,
operational procedures safety measures and any contingency plan. The meeting shall be attended by
the following personnel involve in Anchor Handling

Briefing the Crew - Before carrying out the operation, each Master shall brief their crews on the
operation sequence and procedures. This is to include safety measures and contingency plans, if any.
The crew shall be briefed on safety and operational matters.

Position of AWB and AHT will be available on the AWB in real time. Positioning of anchors will be
carried out by the AHTs. All AHT movements will be monitored from the AWB control room by the
surveyor. Near restricted areas, the precision on the AHTS position will be the one of a DGPS. The
surveyor onboard AWB will log and regularly update the following information:
I. Existing features positions on construction site.
II. Accommodation Work Barge position and heading.
III. AHTs position, heading and anchor dropping position.
IV. Existing features includes platforms, pipelines, wellheads, debris, mooring of other vessels
and/or obstructions as found during pre-survey, and associated safety corridors, will be
plotted on the TMS.

Anchor Holding Test to be done After mooring is completed, a holding test (or tensioning) will be
performed while the Accommodation Work Barge is 100 meters away from the existing platform.
Holding test tension will be at least the maximum tension on each mooring line based on mooring
analysis and it is depending on seabed/soil conditions, the anchor holding test will be extract from
mooring analysis. Anchor holding test/tension will be applied by the Accommodation Work Barge, not
by the AHT. When anchors are in critical positions and dragging might cause risks to existing asset, a
minimum of 30 minutes holding period is preferable.

PTSC OFFSHORE 1 ANCHOR HANDLING PROCEDURE

POS 1 shall invite the Masters of AHT1 and AHT2 on board PEM for pre-job meeting and discussion for
this anchor handling & deployment job. AHTs will be required to come alongside or snatching POS 1
if weather permits. Upon completion of the pre-job meeting chaired by the Barge Master, both AHTS
Masters shall conduct their own meeting among of their deck crew and key officers prior to
commencement of anchor handling operation. JSA is to be consulted.

The Pre-Anchor Job Meeting shall involve Masters from POS 1, AHT1 and AHT 2, Company Man, Client
Marine Rep, positioning surveyors including safety officers, and barge key personnel. The meeting
shall discuss to finalise the hourly schedule and the checklist of the activities to include allocation of
duties respectively within the organisation.

TOWING PROCEDURE - POS 1 Tow from LABUAN ANCHORAGE to D18MP-A PLATFORM, D18
OFFSHORE, SARAWAK by AHT1

POS 1 ARRIVED LOCATION

I. Towing Tug will approach from SOUTHWEST of D18 MP-A (LOCATION : Lat 03°36 19.202 2° 24
44.826 and vessel will manoeuvring at safe speed to position of the platform distance 18m at
210 o(T).
II. Conduct Pre-Anchor Job Meeting with Marine Captain, CSR, Both AHTS Reps and all Parties
involved.
III. Transfer Anchor Handling Equipment to AHTS

Once all Approved and Permitted then, POS 1 will approach to location from SOUTHWEST
side of D18 MP-A Platform

In case of Platform emergency

In the event of fire on-board platform or installation, the vessel to take necessary action instructed by
Offshore Installation Manager or On-Scene Commander.

In case of Adverse weather:

If wind gusting more than 25 knot or swell above 2.5 meters, vessel will:

1. Stop anchor handling activity.


2. Inform AHT to stop operation and stop work until weather subside.
3. No personnel on open deck area.
4. Secure loose items

In case of anchor dragging:

If the anchor drags, redeploy and test the holding capacity. When working adjacent to the platform,
the tension on the anchor wire should not exceed this tested tension. (To Hold Maximum tension 30
Tons for minimum 30 minutes for Anchor holding test)

Retrieval of anchor in case of wire parted:

During the situation of anchor wire parted, vessel will pull out away (minimum 100m from last
structure platform) or at a safe distance. If weather permitted, will instruct the assigned AHT to
recover back the anchor and anchor buoy.

Retrieval of anchor in case of pennant wire parted/entangled:

Vessel will instruct the AHT to recover the anchor by using Grapnel, J-Hook, or 85T shackle.
By using Grapnel or J-hook, it will be connected to the work wire and will be lowered to the seabed.
After that, the AHT will run 90 degrees from the anchor wire direction to recover the anchor. For this
action, it might need several attempts to recover the anchor and it may damage the wire underwater.
For 85T shackle, AHT will alongside our vessel and we will assist to connect the 85T shackle to their
work wire. The shackle will run thru the AWB anchor wire as the AHT move sideways towards the
anchor wire direction. Once the shackle lock at the anchor, the AHT may pick up the anchor to their
steel deck.

Retrieval of running pennant wire (floating buoy):

Vessel will instruct the AHT to retrieve the buoy and pennant wire. The AHT will run & snatching near
the buoy, and catch by using buoy catcher. Once the buoy on deck, they may retrieve the pennant
wire. AHT crew need to carry out inspection and give full report for the condition of the buoy and
pennant wire.

Contingency anchor position:

Contingency anchor position using sea anchor shall be determined when vessel at offshore site which
will be 1NM from platform/location. The criteria of the position are:

1. Clear away from pipe line (check by using BMS equipment for confirmation and TOPO chart)
2. Weather and prevailing wind direction, in case of vessel to be drag away from platform
facilities.
3. Clear from any Vessel, Barge, offshore facilities or Jack Up Rig in Vicinity for AWB contingency
anchor position.
4. To confirm water depth before sea anchor deployment.
