‫تنوض في الليل تدور في هواء ؟ تحس في دقات قلبك ؟ صارلك وفقدت الوعي ؟ يصيرلك ألم في بطة الرجل بعد ما تمشي‬
‫مسافة والزم ترتاح بعدها تكمل ؟‬
2_ Gastrointestinal

‫طلعلك صفير ؟ شهيتك كويسة او ناقصة ؟ نوع أكلك وكم وجبة في اليوم ؟ تقرحات في الفم ؟ حسيت روحك بترجع أو صار‬

‫تحسها زادت ؟ وتعطش هلبا ؟ او تحسها نقصت علي قبل ؟ في دم البول او لونه تغير ؟ في كشاكيش أو رغاوي في البول ؟‬

‫‪( LOC , seizure , headache , loss of vision , double or blurred vision , visual field defect ,‬‬

‫صارلك وفقدت الوعي ؟ صارلك خضة ؟ عندك صداع ؟ نظرك نقص ؟ تشبح في ضبابة أو تشبح في زوز ؟ تحس مجال‬
‫الرؤية نقص ؟ ما تتحملش الضئ ؟ عندك تيبس في الرقبة ؟ سمعك نقص ؟ في طنين في وذانك ؟ تحس الدنيا تلود بيك ؟‬
‫صارلك فشل في جزء من جسمك معاش قدرت تحركه ؟ عندك جهة تنمل أو فقدت اإلحساس فيها ؟ الحظت تغيير في مشيتك‬
‫أو تطيح علي جهة ديما ؟ هل الحظت كالمك تغير ؟ هل فقدت التحكم في البول والبراز ؟‬

‫‪5_ Musculoskeletal :‬‬

‫‪( hair loss , eye ( redness / dryness / blindness ) , rash ( eye / face / photosensitivity ) oral /‬‬

‫عندك تساقط ف الشعر ؟ تحس في عيونك فيهم جفاف أو احمرار أو فقدت النظر ؟ طلعلك طفح جلدي علي عيونك أو وجهك أو‬
‫لما تتعرض للشمس تتأثر بسرعة ؟ عندك تقرحات في الفم أو في المناطق الخاصة ؟ عندك جفاف في الفم ؟ عندك ألم في‬
‫المفاصل أو تيبس في المفاصل ؟ لما تتعرض للصقع هل صوابع ايديك يتغير لونهم يولي أبيض بعدين أزرق وبعدين يرجع‬
‫أحمر ؟ هل تقدر تمشط شعرك وتلبس حوايجك عادي ؟ وتقدر تنوض من علي الكرسي وتركب الدروج ؟ عندك وجاع في‬
‫عضالتك من فوق جهة كتافك ؟ عندك ألم في الفك لم تفتح فمك او تأكل ؟‬

‫‪6_ Endocrine :‬‬

‫‪( hot / cold intolerance , mood , sleeping time , palpitation , diarrhea / constipation , weight‬‬
‫‪loss / gain , acne / hirsutism / stria , polyuria / polydipsia , nocturia , recurrent infection ,‬‬

‫لما يكون الجو صقع نفسك روحك سخون أو لما يكون الجو سخون تحس نفسك صقعان ؟ تحس نفسك بديت مزاجي و تتعصب‬
‫بسرعة ؟ هل بديت ت رقد أكثر من قبل أو أقل من قبل ؟ يطلعلك هلبا حب شباب ؟ عندك زيادة في نمو الشعر زي في الذقن مثال‬

‫وليت شاحب ؟ عندك فشل عام ؟ عندك رغبة تاكل حاجات غريبة زي الفحم والزواق ؟ تحس تركيزك ناقص ؟ طلعلك أي لطع‬
‫؟ نزيف من أي مكان ؟ تعرق في الليل ؟ ألم في العظام ؟ الحظت أي انتفاخ واال طوبة في جسمك ؟‬

‫هل تحس نفسك ديما جوك مليح أو ديما مكتئب وما خاطرك في شي ؟ هل تفكر هلبا في الماكلة أو تحاول ترجع بعد ما تأكل ؟‬
‫هل ديما قالق ومتوتر ؟ هل تسمع في أصوات أو تشوف في ناس وخياالت ؟ هل تحس إنه الناس يراقبو فيك ويتبعو فيك أو‬

‫بدت معاك فجأة الدهشة أو بالشويا ؟ قداش ليها بادية معاك ؟ هل الدهشة تمشي وتجي أو مستمرة ؟ لو تمشي وتجي قداش الفترة‬
‫تقعد فيها داهش ؟ هل الدهشة تزيد لما تتحرك واال تاخذ بردة أو تتعرض لغبرة أو روائح ؟ هل تنقص لما ترتاح أو تأخذ في دواء‬
‫معين موسع شعب مثال أو مدرات للبول ؟ تدهش أكثر في الصبح أو في الليل او مافيش اختالف ؟ باهي تقعد داهش حتي وانت‬
‫مستريح أو تدهش لما تتحرك مثال حتي فالحوش بس أو تدهش لما تركب دروج وتمشي مسافات شويا برا الحوش ؟‬

B _ Chronic dyspnea :
1- OAD ( bronchial asthma , COPD , bronchiectasis )
2- Lung cancer
3- Pulmonary tuberculosis
4- Pulmonary hypertension Main investigations :
5- Plural effusion _ CBC , CRP , ESR , ABG , PFT , X-ray , HRCT .
6- Interstitial lung disease _ D-Dimer , V/Q mismatch , CT angiography .
7- Heart failure _ Sputum for gram stain & C/S & acid fast bacilli .
8- Corpulmonale
_ ECG , ECHO ,BNP .
9- Valvular heart disease
10- Anemia

‫تاخذ في أدوية ضغط ؟ مدرات للبول ؟ حبة تحت اللسان ؟ اسبرين أو أدوية سيولة ؟ أدوية تنقص المناعة أو أدوية لضربات‬
‫القلب ؟ تأخذي في أدوية موانع حمل ؟‬

‫بدت معاك فجأة الكحة أو بالشويا ؟ قداش ليها بادية معاك ؟ هل الكحة تمشي وتجي أو مستمرة ؟ وكان تمشي وتجي قداش تكح‬
‫مرة في اليوم ؟ الكحة جافة أو معبية ؟ واذا معبية قداش كميتها وشن لونها وفيها ريحة ؟ هل تحس فيها زي الرغوة ؟ هل الكحة‬
‫تزيد لما تتحرك واال تاخذ بردة أو تتعرض لغبرة أو روائح أو تجي كل شتاء بس ؟ هل تنقص لما تأخذ دواء معين موسع شعب‬
‫مثال ؟ هل في اختالف بين الصبح والليل واال نفس الشئ ؟ أو تشد عليك مواسم معينة ؟‬

Differential diagnosis :

A _ RES : ( wheeze , chest pain , dyspnea , fever , nasal congestion , nasal drip , throat pain )
1- Upper respiratory tract infection
2- Acute exacerbation of Bronchial asthma
3- Acute exacerbation of COPD
4- Bronchiectasis
Main investigations :
5- Interstitial lung disease
_ CBC , CRP , ESR , ABG , PFT , X-ray , HRCT .
6- Lung cancer
_ Sputum for gram stain & C/S & acid fast bacilli .
7- Pneumonia
_ ECG , ECHO ,BNP . _ 24 ph monometry .
8- Pulmonary tuberculosis
9- Acute Pulmonary edema

B_ CVS : ( orthopnea , PND , palpitation , syncope , Intermittent claudication )

1- Congestive or left side heart failure

C_ GIT : ( heart burn , spicy food , smoker , use of NSAIDs )

1- GERD

D_ Psychogenic

Systemic review main systems are : ( RH , GIT , CNS , renal )

Past history :
Chronic illness ( any lung disease , atopy , heart disease , RH disease , , D.M ,HTN , DVT , GERD )

2- Pulmonary tuberculosis
3- Bronchiectasis
4- Lung cancer
5- Pulmonary embolism

B_ CVS : ( orthopnea , PND , palpitation , syncope , Intermittent claudication )

1- Heart failure
2- Mitral stenosis

C_ Vasculitis : ( purpura , hematuria , polyuria , oral +-genital ulcer )

1- Good pasture
2- Wegner granulomatosis

D_ Blood disease ( pallor , dyspnea , fatigue , headache , loss of concentration , bruise ,

bleeding per orifices , weight loss , night sweating , bone pain )
1- Bleeding tendency
2- Blood malignancy
Main investigations :
E_ Trauma . _ CBC , coagulation profile , blood group & cross
match , CRP , ESR , ABG , X-ray .
F_ Anticoagulant use . _ D-dimaer , V/Q scan , CT angio , Doppler LL .
_Sputum for gram stain & C/S & acid fast bacilli .
_ ECG , ECHO ,BNP .

‫وين مكانه األلم ؟ هل بدي فجأة أو بالشويا ؟ األلم قداش ليه ؟ كيف تحس فيه باهي مثال حاجة تعصر رزينة ع صدرك أو كأنه‬
‫تمزيق أو ينخص نخص أو يحرق فيك ؟ يسمع في جهة رقبتك وإيدك وإال ظهرك األلم ؟ األلم يمشي يجي وإال مستمر وكان‬
‫مستمر كل كم يجي كم يقعد ؟ هل تحس األلم يزيد لما تتعفلق و تتوتر واال تاخذ نفس وإال ترقد مسطح وتكون واكل ؟ واال كنت‬
‫تدير في رياضة ؟ هل خذيت حبة تحت اللسان باش يخف األلم واال أدوية حموضة ؟ وإال تطبس القدام يخف ؟ األلم خالك‬
‫معاش تكمل اشغالك أو ينوض فيك من النوم " كان ليه هلبا " ؟‬

1- IHD / prinzmetal angina .
2- Aortic dissection
3- Pericarditis

B _ RES : ( cough , hemptoysis , wheeze , fever , weight loss , night sweating , fatigue )
1- Pneumonia
2- Pulmoanry tuberculosis
3- Pneomothorax
4- Pulmonary embolism
5- Lung cancer

C_ GIT : ( heart burn , use of NSAIDs , heavy meals , tight cloths )

1- GERD
2- Diffuse esophageal spasm Main investigations :
_ CBC , cardiac enzyme , ECHO , ECG .
D_ Trauma _ Coronary angiography , U/C/E .
_ Coagulation profile .
_ X-ray , CT , PFT , Sputum .
Systemic review main systems are: ( RH , CNS , GIT ,renal ) _ D-dimer , V/Q scan , CT angiography .
Past history : _ 24 ph monometry .
HX of recent infection .
Chronic illness ( heart disease , stroke , any lung disease , RH disease , D.M ,HTN , DVT )
‫عندك فشل في القلب أو مشاكل في الصمامات أو صايرلك جلطة ع القلب قبل ؟ صايرلك أي جلطة ع الرأس قبل ؟ عندك أي‬
‫مرض مزمن في الرئة ؟ أي أمراض روماتزمية ؟ سكر ؟ ضغط ؟ دهون ؟ صايرلك جلطة في رجلك أو في الرئة قبل ؟‬

Drugs ( NSAIDs , nitrate , diuretic , aspirin , anti HTN , OCP if female )

‫تاخذ في مسكنات ؟ حبة تحت اللسان ؟ مدرات للبول ؟ اسبيرين ؟ أدوية ضغط ؟ تاخذي في موانع حمل ؟‬

Symptoms of hypertension ( headache , anxiety , palpation , blurred vision )

Assess complication : - Visual acuity , previous fundoscopy
- dyspnea , cough , orthopnea , PND
- Frothy urine , lower limb edema , previous RFT
- Sensory loss , numbness , limb weakness

B_ Secondary hypertension :
1- cardiovascular & respiratory : ( aortic coarctation , OSA )
2- Endocrinal :
_ Cushing syndrome ( acne , stria , hairstiutism , obesity , recurrant infection )
_ Acromegaly ( change in ring and shoe size , sweating , visual field defect )

nausea , vomiting , abdominal pain , etc )

 Example of case presentation

Mr. Ahmed Ali Mohammed , he is 65 years old , unemployed , he is Libyan and lives in Tajura ,
Mr. Ahmed is married and has 4 children .

Mr. Ahmed admitted 2 days back form the OPD , After he complained of lower limb edema .

The lower limb edema started since two months back with gradual onset and progressive course ,
the edema is bilateral and reached his knees now , it was aggravated by long standing and relieved by
elevation of legs . Mr. Ahmed admit that his shoe and socks leave a mark on his legs and denies any
redness or hotness or any edema in other site of his body .

Regarding systemic review , he was compline also of dyspnea which started before his edema and it
was occur even during rest , orthopnea , PND , but no cough , wheeze , chest pain , syncope or
intermittent claudication .

While GIT system was unremarkable .

Or regarding GIT there was no jaundice , oral ulcer , dysphagia , odynophagia , heart burm ,
abdominal pain , abdominal distention , diarrhea , etc .

Other systems in the same way “ remember that you have to mention the positive symptoms of
each system before the negative “ , and try to organize the systemic review regarding each compline
specifically .

In Mr. Ahmed past medical review , he has no any previous admission to the ward or to the ICU .
Mr. Ahmed is known case of DM since 15 years with poor follow - up and on insulin three times per
day , his last HBA1C is 10.2 ,and he did not seek any medical advice from the ophthalmologist or
nephrologist before , he checked his blood pressure and his lipid profile before , both were normal as
the patient said .
He is not known case of heart failure , liver failure , renal failure , or hypothyroidism , and has no
previous attack of DVT .

He doesn’t take any other medication rather than his insulin like CCB , NSAIDs , Steroids , etc .
He didn’t underwent any surgical intervention before .

Mr. Ahmed lives in a flat house in Tajura , he has a car , he is smoker since 45 years back , 1-2 packets
per day , non – alcoholic , and has a good income that can cover any medical services that he will
need it .
