located in Tori-Tori, Anda , Pangasinan containing an area of 7,705 square meters. That we caused the
subdivision of the above-described property into six (6) lots which as follows;

1. To: DEBORAH CAROLINO AQUINO - Lot-5101-C containing an area of 1,322 square

meters.

2. To: HEIRS OF EXPEDITO CAROLINO - Lot-5101-B containing an area of 1,662 square

meters.

3. To: DOMINGO O. CAROLINO married to Trinidad D. Carolino - Lot-5101-A containing an

area of 4,564 square meters.

4. To: RIGHT OF WAY - Lot-5101-D containing an area of 157 square meters.

That we hereby confirm the foregoing subdivision survey to be true and correct to the best of our
knowledge and information.

That we are executing this affidavit in order to indicate in detail our ownership over our individual lots
as appearing above and/or correct any erroneous entry in the title relative to our individual lots.
