DATE : Week 1:

The Municipality of Placer Surigao del Norte organized and conducted its
1st Session of the Community Based Community-Based Drug Rehabilitation
Program (CBDRP): General Interventions for Persons Who Use Drugs (PWUDs) of
Barangay_______________. There were a total of ___________ PWUDs who attended
the session. For this session, the Rural Health Unit conducted random point-of-care
drug testing wherein all PWUDs had NEGATIVE Results for the use of
methamphetamine and tetrahydrocannabinol (THC). Aside from the point-of-care drug
testing, upon the arrival of the clients they are advised to sign the attendance sheets &
undergo the Profiling, after which the initial Screenings follow and this was conducted
by our trained DOH Accredited Physicians.

The program is facilitated by a facilitator outsourced by the LGU. Our CBDRP

Facilitator Ms. Shiela Mae D. Banzon. Facilitated the discussion pertaining to Objective
& Guidelines of the CBDRP Program, Signing of Inform Consent & short discussion of
R.A 9165. The Clients are eagerly listens with the discussion and participated during
workshop until the end of the session.

Activity : Nature of Addiction & Crises Management

DATE : Week 2:

At about 4:00 P.M., at the Brgy Multipurpose Hall ____________Surigao del

Norte conducted its 2nd (Second) Session for the Community Based Community-Based
Drug Rehabilitation Program (CBDRP): General Interventions for Persons Who Use
Drugs (PWUDs) of Barangay _____________. Lectures on the Science of Addiction
and Recovery & Types of Drugs are the highlight of this week. It is part of the program
to provide openness and compassion to clients by helping the PWUDs, and their
families understand that Addiction is an illness, and that recovery brings healing to that
illness. Describing the role of the Brain, the cause and effect of addictive substances
and that was conducted by the CBDRP Facilitator to the PWUDs.

The importance of this session is also aimed to help the PWUDs identify how
their use of substances has affected their families/peers and determine how family
members can provide support for their recovering family members.
In addition, an activity was conducted wherein PWUDs were given the chance to
realized how greatly affected by their use of illegal substances in the past and to an
answers sheets were given as part of the session. After that a sharing of the
volunteer clients was performed as part of the process. The session ended at around
6:45 P.M. on the same place and date.

Activity : Family Session and One on One Session

DATE : Week 3

During the 3rd week of the General Intervention sessions aim to assess and
assist clients on how they incorporate learning delivered through lectures leads to
productive behavior, improved thinking, and positive coping mechanisms.

We have Special Participation from an Outsource Facilitator of the LGU from

Cagayan de Oro City Mr. John Gabriel Lasquites, an International Certified Addiction
Professional who conducted Individual Sessions to provide an avenue for clients to
freely express themselves without the fear of being judged and be able to seek help in
addressing their internal needs. It is also done to periodically assess clients’ progress
and plans for their reintegration. Also he conducted a lecture topic pertaining to
Families and shared his personal experience about Addiction and these meetings pave
a healthy way to explore the emotions of clients with people who understand what they
are going through. It also provides an avenue for clients to share experience, strength,
and hope with co-members. Through the sharing of experiences, they're able to offer
support, encouragement, and comfort to the other group members, and receive the
same in return.

Activity : Joint Physical Activity and Graduation Ceremony

DATE : Week 4

On Week 4 of the Batch 2 of implementation of the Community Based Drug

Rehabilitation: General Intervention, at Barangay _________________the Local
Government Unit of Placer, Surigao del Norte conducted a joint Physical Activity as part
of the fun and recreation at the same time conducted a Commencement Exercises for
the (CBDRP): General Interventions for Persons Who Use Drugs (PWUDs) of
Barangays There was a total of _________ PWUDs who have participated and
completed the Program. For this session, the Rural Health Unit conducted the last
random point-of-care drug testing wherein all ___________ PWUDs had NEGATIVE
Results for the use of methamphetamine and tetrahydrocannabinol (THC). After the
point-of-care drug testing, This was followed by the Commencement Exercises Proper.
In the commencement exercise, inspirational messages were given by the cooperating
agencies to the PWUDs. The PWUDs also made their Pledge of Recovery and
Certificate of Completion were given to the PWUDs who completed the program.
