Instruction:

A total of 20 fire doors are installed in the chemical plant of Chemical Industries Sdn Bhd
which manufactures ‘speciality’ chemical that is flammable. The fire doors are single leaf
with dimensions of 2137 x 1050 mm and 80 kg. Three hinges are installed for each door
with positions as shown in Figure 1. Each hinge has a label of [3-7-4-0-0-0-0-11]. Each
closer has a label of [4-8-3-0-1-3]. Based on 3 years record of the reliability and asset
management team, the fire door has been classified as a ‘bad actor’ and hence the need to
perform root cause analysis (RCA) has been put forward to the plant manager.

Among the identified damaged components or observations at the fire doors were:
• Broken door dampener.
• Broken mortise lock.
• Slamming of fire door due to non-functioning door closer which has even led to
damage of door frame and panel.
• Rubbing of fire door to the floor.

You are required to perform the RCA in a group of 4-5 students and submit your report. You
are also required to do a physical group presentation.

Guideline for the report (100% consist of 10 marks):


Report [70] (5-10 pages):
a) Cover page consists of all members in the group (with student matric number)
b) Introduction (background of the problem)
c) Data collection and presentation of data.
d) Results (data analysis) and discussion
e) Conclusion & recommendation
f) References

Presentation [20%] (5-10 min)

Slides [10%] (5-10 slides)

Format for Preparing Report:


a) Use font size 12 Arial with 1.5 spacing.
b) The margins are as follows – top and bottom margin 1.00”, left margin 1.25” and
right margin 1.00”.
c) Use ring binding for the report with transparency cover on both the front as well as
the back.
d) The report must be written in English.
e) The report must have a front page; members name, contact number and profile
photo for each member; table of content; body text; conclusions; references and
appendices.
f) ZERO mark will be given if you got caught cheating and plagiarizing materials from
friends.
g) If you want to include materials from internet, journal article or textbooks, please
include citations clearly.
h) All figures and tables must be numbered accordingly, have titles or description and
referred to in the text.
i) All marked reports will not be returned and please make extra copy for yourself.
j) All the supplementary data and analysis using software or spreadsheets need to
be submitted in the form of softcopy (ULearn).
