AFFIDAVIT OF INCOME

I, RUSTUMME MIRELLE B. REYES, Filipino, of legal age,

single, with residence address at Brgy. Sto. Rosario, Baybay City,
Leyte after having been duly sworn in accordance with law, do
hereby depose and say:

1. That I am presently employed as a Local Government

Operations Officer II at the Department of the Interior
and Local Government – Regional Office VIII, Kanhuraw Hill,
Tacloban City;

2. That my monthly gross income is thirty-three thousand

three hundred and twenty pesos only (₱33,320.00),
inclusive of Personnel Economic Relief Allowance (PERA);

3. That I executed this affidavit in good faith to attest the

truthfulness of the foregoing statements to support my bank
loan at the Bank of Philippine Islands Main Branch, Justice
Romualdez St., Tacloban City, and for whatever legal
purpose this may serve.

AFFIANT FURTHER SAYS NONE.
