BOARD RESOLUTION

I, Dominga M. Orencia, of legal age, widow, Filipino and residing at block 10 lot 75, Phase 1A, Suburban,
San Jose, Rodriguez, Rizal, after having been duly sworn to in accordance with law, do hereby depose
and say

1. That, last march 23,2022, we the officer of the Immaculate Conception Chapel have a
meeting held at said chapel with our Parish Priest Rev. Fr. Michael B. Balatbat at Maria Ina
ng Kapayapaan Church at kasiglahan village, San Jose, Rodriguez, Rizal.
2. That one of the topic of the meeting is to continue the savings account of the said Chapel,
Immaculate Conception at Rural Bank of Montalban, Inc. – Head Office.
3. That the two (2) previous signatories Rev. Fr. Bonifacio Chavez and Goerge Haresco has
been changed.
4. That our present two (2) bank signatories are Rev. Fr. Michael B. Balatbat – Maria Ina ng
Kapayapaan Parish Priest and Rebecca G. Legaspi – Immaculate Conception Chapel
Coordinator.
5. That this statement is needed to continue Immaculate Conception Chapel savings account to
Rural Bank of Montalban, Inc.
6. That I am exerting this Board of Resolution to attest the truth of the foregoing statement.
