Dear Sir/Ma'am

Good day, I am Lemuel A. Crispino Jr. from FM - 3b. I am sorry Sir/Maam for forgetting
the evaluation. I know that I have a responsibility to evaluate the professor and it is need for
clearance. We know that we are on distance learning Sir/Maam and some of us are still adjusting
for the connection or internet service. And that time I have a lot of activities to be done and I am
still using only phone and prepaid loads to attend my classes and create my activities. And the
evaluation is January 3, 2022, when the year 2021 ends my brain is unstable not because I’m
celebrating alone the Christmas and year end but because my family is facing a lot of problem in
the province (Negros Occidental) they are also hit by a storm and the ceiling of our house in
there is destroyed by a storm rai and the electricity in there is gone until Feb. And that gives me a
lot of thinking about what is the situation of them in there. That’s result on me of not doing of
some my activities in January 2022, but I cope up because I need to but it’s too late because I
forgot the important part of being a student and that is to evaluate the professors. Again, I am
Lemuel A. Crispino Jr. from Fm 3b saying Very Sorry to Sir/Ma’am for forgetting the
evaluation.
