INSTRUCTIONS FOR RISK ASSESSMENT

1 Use the checklist in the Hazards Inventory Worksheet to identify categories of hazards that staff in your workplace are exposed to by clicking in the checkboxes
2 Access Risk Assessment forms for specific hazards using links in the Hazards Inventory Worksheet
3 Complete risk assessments for other general hazards using Generic Risk Assessment Form. See example below
Refer to " Risk Management Guide " for the risk assessment process
4 You can print out the form and hand write or fill in the form electronically. Keep a signed copy as your record.
