Centre of Gravity
Obvious Estimated Determined by drawing

Description of Lifting Operation:

To lift pump approx.weight 200kg. Use 2 weebing sling 1T x 2m, 1 weebing sling 1T x 1m, 1 weebing sling 1 T
x 6m , shackle 3 x 2T , master link , 2 chainblock 1T

Possible Consideration (not exhaustive)

(tick if relevant and address each point in “Step-by-Step” section below)

Weight not verified Lifting of chemicals Conflicting tasks in area

High centre of gravity Load on pallet requires securing Dynamic factors involved

Stability of load Sea fastening removed Hazards to personnel in the area

Restricted headroom or confined work

Awkward size/shapes/sharp edges Communication requirements
area

No dedicated lifting points on the load No lifting points directly above the load Adequate lighting and visibility

No certified suspension point for the Appropriate and correctly installed lifting Is the use of tag lines required? Then
lifting equipment equipment and accessories consider personnel positioning

Route to be Travelled and Laydown Area

(if you can’t answer either, address in the “Step-by-Step” section) YES N/A
1. Are the route and laydown area clear of obstructions?

2. Is the laydown/landing area adequate in terms of size and load-bearing ability?

3. Is suitable packing available for protection of the load, lifting equipment, slings etc?

4. Have barriers been positioned to prevent access by unauthorised personnel?

5. Have you confirmed that the laydown area is within the operating limits/radius of the equipment?

6. Have environmental conditions been considered with regards to the safety of the lifting operations?

7. Will the Crane Operator be able to see the Banksman throughout the operation, or has another suitable,
risk-assessed means of communication been checked and made available (e.g. radios)?

List of Guidewords for Hazard Identification
This list of guidewords for the identification of hazards associated with the use of lifting equipment is divided into five
sections, i.e. Nature of Load, Environment, Strength, Stability and Position.
The guidewords shall be used in conjunction with the initial lifting plan and the preliminary equipment selection to identify
possible hazards associated with the specific lifting operation.
For each hazard identified the review team shall record actions necessary to eliminate or reduce the likely impact of the
hazard.
It is not intended that this list of guidewords is exhaustive and the review team may identify additional items requiring to be
considered during the process of hazard identification.

Nature of Load

Size Weight Shape

Centre of Gravity Stability of the load Protected / Unprotected

Hazardous / Non-hazardous Dangerous Snow / Ice build-up
