Approvals
NOTES: 1 - The Lifting Plan shall be approved by a Competent Lifting Person prior to the execution of
the lift(s).
2 - The execution of the lift(s) shall be in complete compliance with the Lifting Plan and
applicable local regulations and international standards.
Checklist (Complete the checklist to ensure that a safe lift is planned)
1. Prior to the lift(s)
Validity of Crane Operator and Rigger 3rd Party certificates and/or licenses has been verified.

The correct Work Permit has been raised, with pertinent documents attached and properly authorized.

The weight and center of gravity of the load has been confirmed known.

The lifting gear (slings, shackles, etc.) has been inspected by a Competent Lifting Person and is found
fit for purpose.

The validity of the lifting gear (slings, shackles, etc.) 3rd Party Test/Inspection has been verified.

The TBT and site visit has been conducted by the Supervisor/Foreman.

The working area has been barricaded; safety signs are sufficient and visible.

Crane safety devices has been checked and found operational.

Boom angle, boom length, lift radius and crane capacity has been determined.

Ground/soil/pavement has been confirmed to have capacity to support the imposed load.

All outriggers has been fully extended, jacks deployed and resting on sole boards/plates.

Crane has been leveled; tyres are clear off the ground.

Crane has been set up in accordance with Manufacturer's operating instructions.

Crane hook is directly over the load's centre of gravity.

Workers at a height > 1.2 m, while attaching shackles/slings, has been provided with ladders and
full-body harnesses.

All obstacles and obstructions have been identified.

Proper rigging arrangement has been confirmed; tension has been applied on the lifting gear.

Lifting Supervisor and the crew has been briefed and are fully aware of the Emergency Procedure
in case of crane loss of power, malfunction or falling load.

Crane Operator has been reminded to remain alert and never to leave the controls any time during
the lift.
Readiness of Crane Operator and Rigger to start the lift has been confirmed.
A signal method has been determined between the crane operator and the signalman.

2. After the lift(s)

The lifting gear (slings, shackles, etc.) has been removed, cleaned and stowed properly.

Barricades and safety signs has been removed and stowed properly.

1. Is everyone aware of and do they fully understand the lifting and hoisting procedures applicable to the lift?
2. Has everyone attended the toolbox talk?
3. Has a pre-use inspection of the Lifting Equipment been carried out and are the Lifting Accessories tagged or
marked with: Safe Working Load

4. Are all safety devices working?
5. Does everyone know the Person-In-Charge of the lift?
6. Is everyone competent and aware of his or her tasks?

9. Is the lift area controlled and is everyone clear if the load falls or swings?
10. Are signalling methods and communication agreed and clear to you?
