Subject: Dropped Object Incident Involving IBOP Crank Assembly on TDS-8SA Top Drive
Product: All National Oilwell Varco (NOV) Top Drive Systems, with the exception of the TD, HPS,
PS/PS2, and TDX-1250 Product Lines

Assemblies: IBOP Valves (Various Part Numbers)
Issue: NOV has received a report of a second incident where a Cam Follower (P/N 79824)
failed following rough drilling operations. Unlike the first incident where the components
remained captive the latest occurrence of this failure allowed the Spacer (P/N 99168) to
fall to the rig floor. The cam follower and spacer are components of the IBOP Crank
Assembly (P/N 30173893 & 98898), as shown in Figure 1. No injuries were reported
as a result of this incident.
Solution: Based on the investigation of the incident it has been determined that damage was
caused by rough drilling operations which led to the eventual failure of the cam follower.
NOV recommends that a post-jarring inspection be performed on the Top Drive per
previously released Product Bulletin TDS-93-1, as the potential for damage to the Top
Drive during jarring or rough drilling operations is considerable.
The frequency that the IBOP Crank Assembly should be inspected is weekly or as
stated in the product specific Service Manual. It is also recommended that inspection of
the IBOP Crank Assembly be performed prior to every reinstallation following IBOP
change outs.
