We would like to request to update the BIR Form No. 2303 or Certicate of
Registration of MERIDIAN CAFÉ. We would like to include Form 1601-C
(Monthly Remittance Return of Income Taxes Withheld on Compensation) and
Form 1604-CF (Annual Information Return of Income Taxes Withheld on
Compensation and Final Withholding Taxes) in the Certificate of Registration. The
reason for the update is for us to claim as a deduction the compensation of
MERIDIAN CAFÉ’s employees.

We like would also to include form 1701 (Annual Income Tax Return for Self-
Employed Individuals, Estates and Trusts) and 1701-Q (Quarterly Income Tax
Return for Individuals, Estates, and Trusts). The business is situated in Gaisano
Grand Mall of Digos, Quezon Ave., Digos City.
