PRECAUTIONS: Inform to all operation personnel’s , contractors and security Guards

Now undertake the JSA and record in the table below. Use prompts on JSA Prompt Sheet and Hazard Checklist PA-HSEC-02-040-002-F03.
Part 3: Contingency Plans. Standard PPE,S : First aid kit must be present at the area and the Hse personnel is the first response in case of
emergency

Part 4: Review. After the job has been completed, JSA leader should note: any hazards, which were identified in the original JSA? Any practices
that need to be captured in the OMS? List action items, changes, and transfer any changes to the Change Management system.

1 Work preparation and mobilization Unauthorised equipment/tools brought out to Prior to mobilizing to site ,all equipment shall be

Four permitter’s lights with spotters will be
deputed.
The area will be barricade with RT tape.
2 Storage and transportation of source. Source stolen DEL to maintain security precautions to prevent
armed theft of source as follows.
The source will be locked in its designated storage
area immediately after use.
 Poor illumination Use IS rated potable lights
 Manual Handling Use proper manual handling-Conduct tool box

3. Set up for RT activity SIMOPS / inadequate barrication around test Ensure barrication is installed at a distance
point calculated where the radiation intensity should not

In adequate preparation in the event of A drill shall be conducted prior to job execution at
