Task 1: Emergency response

Question 1
As Health and Safety Adviser I managed the emergency by performing following actions:
 First of all, I took charge of accident location and quickly responded to the
emergency as there was no responsible person.
 Disperse the crowd from accident premises so have more space to give
emergency procedures specially the bystanders and secure the area.
 Seek Assistance from manager and team leader but they are not available as
they already left cinema.
 There was no trained first aider from staff as certified first aider I seek for first
aid kit but it was not properly maintained only few bandages and plasters were
available.
 Properly Examine the Victim to know the condition of injuries and level of
damages.
 Called the Emergency services for help and better treatment of injured personal.
 Consoled the injured worker to make him feel comfortable and conscious by
telling him that ambulance is on its way.
 Cleared and closed the entrance hall of cinema and Evacuated the public from
fire exits of each auditorium make sure there is no visitor in cinema.
 Ambulance reached the cinema and provided necessary paramedic treatment
before leaving for hospital. Later came to know that worker’s one leg was
broken, abrasions and bruise on body.
 Normalised the workers who are in condition of trauma and shocked after
witnessing of accident also suggested them to leave for their home if they want
to.
 Gathered the information of accident from witnessed workers in form of written
documents for investigation purpose.
 Tired to telephone Manager to inform of cinema by calling him on phone but can’t
connect so dropped the voice mail message.

Task 2: Culture

Question 2
Health and safety culture can be described as mutual health safety attitudes, values,
believes, expectations, competencies, and behaviour pattern. Following are the negative
culture indicators for health and safety at waterfront cinema:
 In waterfront there is Lack of competency is shown by management towards
health and safety policy.
 Poor Housekeeping and improper maintenance of cinema as there was hazard
of slip and trip cause of littering on the floor and over flowing waste bins and no
proper waste management system.
 Unprofessional and rude behaviour with Customers and staff treating them
harshly showing bad attitude and using phone during working.
 Workers are afraid of team leader which shows influence of peer and perform
risky activities in cinema.
 There is Bully and harassment culture as worker are force to perform activities
which they deny to perform and team leader mocked them.
 Health and safety policy is out dated as risk assessment was carried out two
years ago also not revised.
 No display of health and safety policy and no safety signage in cinema for
guidance of workers and public.
 No commitment of health and safety by management in cinema as the manager
don’t prioritized the health and safety system in work place.
 Lack of Cooperative coordination and communication with workers about
workplace hazards.

 improper consultation of workers in cinema that show negative health and safety
culture.
 There is no proper risk assessment of hazards as High accident rate of similar
nature incidents were observed in cinema two months ago.
 No proper arrangement of first aid resources and emergency response
system was available to handle emergency conditions in cinema.
 Unhygienic workplace environment in cinema and lack of Welfare facilities for
public and staff.
 Lack of Assistance from management during the accident such as to barricade
the area or response to emergency.
 Equipment is poorly maintained and not up to mark according to safe working
practices as inspection of equipment is not carried out in cinema regularly.
 No investigation and written records of previous incidents were there for better
assessment to control future hazards as manager thinks its waste of time
 There were Lack of resources for effective and safe completion of task.
 Health and safety is not priority for cinema management no committee
meetings are arranged for there to know workers issues and problems.
 Workers feels pressurized and threatened by the manager and being afraid of
dismissed from the work.
 Staff Turnover is higher creating a sense of job insecurity amongst the workers.
 There was no proper defining of Roles and responsibility for workers to perform
their own job.
 Non-compliance with health and safety as manager was no interested in
implementation of health and safety procedures.
 No Supervision of workers was there to counter work related hazards during daily
activities.
 Training to Workers was not adequate enough as they were unable to handle
panic situation such as accident/incident.
 Staff complaints are being ignored by the management and condition of
equipment are not up to mark to complete task.
 There was no proper equipment and precautions were adopted for working at
height as stepladder was unable to fully opened and wobbled.
 No learning from previous accidents, no effective actions are taken in account
and continue doing those unsafe working methods in workplace.
 Workers are unware of health and safety system and unware of basic right of
provision of PPE and proper first aid kits for workers
 Risk assessment is not suitable and efficient as risk assessment was of old
ownership of cinema with different name and address.
 Vulnerable workers are doing part time job in cinema and children are also there in
area which is alarming hazard.

Task 3: Health and safety management systems – awareness and


communication

Question 3
If there is regulated health and safety system in any work environment everything will be
according to standard and in line but in waterfront cinema following are the requirements
to communicate effectively with workers to have efficient health and safety system.
 Unawareness of health and safety management system in cinema, that should
have been adopted in such case of emergency.
 Lack of Reporting system regarding health and safety of workers in cinema
 No induction training to vulnerable workers and temporary workers as, it is
compulsory to inform them about health and safety measurements and policy
 Risk assessment is outdated and not reviewed workers are unaware about the
hazards they are being exposed to due to lack of awareness

 Lack of communication to workers about potential hazards and how to overcome
them
 Health and safety measurements are not displayed and neither communicated
to workers that caused this incident in waterfront cinema.
 Lack of safety measurements that are to be adopted while working on elevated
platforms. Worker were not trained or communicated thoroughly.
 Management is incompetent and unaware of documentation about the health and
safety, that are to be presented for audit.
 Lack of training for workers at cinema, such as how to work at height, first aid
training in case of an accident, no housekeeping training and instructions to
workers are not provided.
 Lack of record of current and past incidents, no procedure beings followed in
case of an accident/incident at the cinema.
 Lack of communication, coordination and cooperation between workers and
management which is their failure.
 Maintenance and repair works of defective equipment is not taken seriously as
workers are continuously complaining about it and this is a failure of reporting
system and case accidents.
 Pervious accidents are ignored by the management as there is no effective
reaction from the management
 Workers are being threatened by management to do their job regardless of
circumstances otherwise they will be fired.
 Vulnerable workers are involved in high risk activities and no training is being
provided to them ,as they are being are bullied and harassed at workplace.

Task 4: Top management commitment

Question 4 (a)
Following are effective commitments from top management of FCC to reduce incidents at
workplace.
 There is an autonomous, centralized health and safety department in FCC to
ensure the health and safety of workers at workplace.
 A Director of health and safety department is responsible for developing health
and safety strategy for their workers.
 Regional managers are responsible to ensure implementation of strategy in
chain of cinemas. This means that they are aware of their role and
responsibilities.
 Health and safety advisors travel across the country to audit the chain of cinemas
and ensure the health and safety arrangements are implemented.
 Experienced personnel are available in every department all of them have
minimum three years’ relevant experience in health and safety.
 All the safety workers are qualified professionals with appropriate knowledge.
 Roles and responsibilities are assigned to every worker at workplace and in
cinema chain.
 Top management claims that they are committed to achieve standards of health
and safety which is ISO 45001:2015.
 FCC help local cinemas and provide consultations to achieve health and safety
standards.
 FCC regularly perform audits in their health and safety system which shows their
commitment.
 They also arranged audit for waterfront to ensure that they have better health
and safety system.
 Advisor checks the documentation of waterfront which shows their competency
of their work.
 He controlled the situation of accident, played a leading role and performed
interviews to collect information for investigation.

Question 4 (b)
After the serious injury at waterfront cinema, it is now expected that labour inspectorate
will visit the waterfront cinema and following actions could be taken by labour inspector.
 Labour inspector can provide effective advice relating to health and safety
standards and laws and regulations.
 He can Rectify the defects and violations which are occurring in the cinema by
taking action on them.
 He can report to senior competent authority or law enforcement agency so
they can take further legal actions.
 Legal actions may also be taken against the organisation if they are not complying
to health and safety rules and may face fine or even imprisonment.
 Labour inspector can take action according to national or labour laws against the
violation by organisation.
 Improvement notice or probation notices may be issued to cinema to overcome
defects in workplace.

Task 5: Administrative control measures

Question 5
Following are the administrative control measure that can be taken to prevent recurrence
of such incidents in the cinema.
 Analysing and updating existing health and safety policy and overcoming current
shortcomings for prevention of future accidents.
 Allocation of supervision to workers on any activity that is undergoing in cinema
such as work at height.
 Inspection and maintenance should be done to ensure safety of equipment and
workplace in order to avoid such accidents in future.
 Installation of Safety signage should be deployed around activity area to avoid
public collision.
 Health and safety training relating to job such as work at height etc. should be
provided to workers and instructions should be given properly to complete the task
safely.
 Authorized responsible, competent and physical fit workers should be assigned
for job, use of proper equipment should be ensure and workers involved should be
aware of their surroundings,
 Supporting staff should be present to help and assist the worker for the job to be
done more efficiently.
 Safe working methods should be displayed, communicate to workers and must
be adopted.
 Work at height is a high risk activity, so it should be requiring a permit to work
which should be granted after ensuring that all health and safety measurements
are taken.
 Emergency response should be quick, well organized and up to the standards.
 Arrangement of first aid is compulsory at the workplace with workers trained
sufficiently to provide first aid in case of any accident.
 Complaints and issues that the workers are facing must be heard and
addressed to ensure smooth work flow.
 Risk assessment should be suitable and sufficient and efficient for workplace
 Worker should be encouraged to report incidents and near misses so that they
can be avoided in future.
 Worker should be provided proper resources to avoid accident at workplace and
warning tapes must be deployed to keep public out of work zone.

Question 6 (a)
Worker along with their jobs bring different factors of individual characteristics like habits,
attitudes and personality which greatly influence the working environment. Following are
the individual human factors that are likely to influence the behaviour of the injured
worker.
 Young workers are likely to be more vulnerable due to age factor, lack of
experience
 Workers are not competent enough to carry out work at height
 Fear factors such as height phobia etc. may also play role.
 Easily influenced worker by team leader
 Training, knowledge skillset and experience may not be up to the standard in
order to perform skilful job.
 Worker are unable to perceive risk.
 Physically immature, unfit and weak to perform the heavy work.
 Lack of self-confident to do the job efficiently.
 Prone to taking unnecessary work pressure.
 Performing high risk activities unsafely is normalized.

Question 6 (b)
The characteristic or nature of specific job or task also influence the behaviour of workers
towards health and safety. Following are the job factors likely to have influenced the
behaviour of the injured workers
 High risk activity is hazard to human life
 Urgency of completing a task may have influenced worker to work under stress.
 Lack of supporting staff during work.
 Poor maintenance of equipment and regular usage without repair shows
negative behaviour.
 Work being performed at a public location with no cordoning off of public.
 Instruction and information regarding safety and job is not available in workplace
for worker
 The task could have been done at any other time when public was not involved.
 The workers were unable to perform task safely due to lack of written
procedure for work at height.
 The signage display for public to be careful while crossing hallway where task at
height is being carried out

Task 7: Risk evaluation

Question 7 (a)
By using matrix and confirm that the risk level was not acceptable when the ladder was
used for banner hanging activity following are few evidence collected form the scenario
that can define risk is unacceptable
 Work at height activity ladder is old and damaged such as locking device is not
working.
 Previously accidents were involving the very same ladder.
 Lack of supervision during work, unsafe wobbling of ladder and no support to the
ladder caused the accident.
 No barricaded of the area while working on the ladder
 Maintenance of ladder was not performed
 Worker was incompetent to perform the job as he was lacking training and skill
required.
 No emergency response was provided after accident.
 Previously injury was including fracture of arm and now accident caused
abrasion on right hand.

As per information collected from the scenario mention above the likelihood risk rating is
on 3 (very likely) as chance are more in workplace
Severity level on height is high severity is 2 (first aid injury)
Risk =Likelihood x severity
Risk = 3 x 2 = 6
6 is lay in high level as per table and in red category of table and risk is not
acceptable

Question 7 (b)
By using matrix and confirm the risk level of banner hanging activity is acceptable, if
mobile scaffolding was used. following are some reason that risk will be acceptable after
use of mobile scaffolding in workplace.
 Mobile scaffolding has a proper working platforms for height as it has edge
protection system.
 Mobile scaffolding has locking mechanism on tyre so it doesn’t not wobble
around.
 Out triggers on the mobile scaffolding prevent toppling over.
 Mobile scaffolding doesn’t not require additional support, so it can be used un
open spaces effectively.
 It has capacity to store tools and materials safely.
 Can be used efficiently for multiply tasks as it’s easy to move around.
 By using mobile scaffolding and above information we can presume that likelihood
would be 1 (unlikely) and severity rating as per above information is 2 (first aid
injury)
 Risk = Likelihood x Severity
 Risk= 1 x 2 = 2
 Risk is 2 which is low as per table and acceptable
