5- Conducting Daily toolbox talk- Conducting daily toolbox talk or pre-task briefings prior
to start of work could have help to prevent the accident.
6- Conducting proper induction training

Task 5: Identifying the influence of organisational and job factors

Question 5 (a)
Organisational factors that might have negatively influenced health and safety behaviour
at the nursery include.
1-Lack of clear health and safety policies and procedures: The health and safety policy
found in the folder was outdated and signed by the previous owner. This suggests that
there may not be a clear and up-to-date policy in place, which could lead to confusion
and inconsistency in health and safety practices.
2-Inadequate health and safety training and induction: The apprentice who had the
accident on their first day of work was not properly informed about the rules and
regulations regarding the presence of two workers in the garden area. This indicates a
lack of proper training and induction for new employees, which can contribute to unsafe
practices.
3-Insufficient staffing levels: The fact that the nursery was understaffed on the day of the
accident suggests that there may be a recurring issue with staffing levels. Insufficient
staffing can lead to increased workload and pressure on employees, which can result in
shortcuts being taken and increased risk of accidents.
4-Lack of communication and consultation: The DM mentioned that they were not aware
of the review and had not prepared any additional health and safety documentation. This
indicates a lack of communication and consultation between the DM and the senior
management team, which can hinder the implementation of effective health and safety
measures.
5-Failure to address equipment maintenance and replacement: The climbing frame in
garden B was in a state of disrepair, with a missing rung and loose rung. The previous
owner had refused requests for a new climbing frame, indicating a failure to address
equipment maintenance and replacement. This can increase the risk of accidents and
injuries.
6-Absence of first-aid provision: The fact that there was no first-aider on site when the
accident occurred highlights a lack of provision for immediate medical assistance. This is
a serious health and safety oversight that could have potentially worsened the
apprentice’s injury.

7-Lack of visible health and safety reminders: The absence of health and safety posters,
policies, and emergency notices throughout the nursery suggests a lack of emphasis on
health and safety awareness and reminders.
8.Inadequate management of recruitment and staff turnover: The DM mentioned that
they were constantly replacing workers, indicating a high turnover rate. High staff
turnover can disrupt continuity and consistency in health and safety practices, as new
employees may not be adequately trained or familiar with the nursery’s health and safety
procedures.
9-Failure to address worker concerns and requests: Worker A mentioned that they had
been requesting a new climbing frame for over a year, but the previous owner refused.
This failure to address worker concerns and requests can lead to frustration and a lack of
motivation to prioritize health and safety.
10-Lack of a proactive approach to health and safety: The DM’s comment about having
more important tasks to focus on suggests a lack of prioritization and proactive approach
to health and safety. This attitude can contribute to complacency and a lack of
commitment to maintaining a safe working environment.
11-Inadequate supervision and oversight: The fact that the apprentice was put in charge
of a group of children on their first day of work without proper supervision indicates a lack
of adequate supervision and oversight. This can increase the risk of accidents and
injuries.
12-Lack of regular health and safety inspections: There is no mention of regular health
and safety inspections being carried out at the nursery. Without regular inspections,
potential hazards and risks may go unnoticed or unaddressed, leading to increased
health and safety risks.
13-Lack of a safety culture: The absence of health and safety posters, policies, and
emergency notices, as well as the lack of visible reminders, suggests a lack of emphasis
on creating a safety culture within the nursery. A strong safety culture encourages
employees to prioritize safety and follow proper procedures.
14-Lack of accountability: The fact that the DM did not have any additional health and
safety documentation prepared for the review indicates a lack of accountability for health
and safety management. This suggests that health and safety responsibilities may not be
clearly defined or enforced.
15-Lack of communication and coordination between management and staff: The DM
mentioned that they were not aware of the review and did not have any additional
information prepared. This suggests a lack of communication and coordination between
the senior management team and the DM, which can hinder effective health and safety
management.
16.-Lack of a proactive approach to risk assessment: The generic risk assessment found
in the folder only referred to the building and did not address specific hazards or risks in
the nursery. This indicates a lack of a proactive approach to risk assessment, which is
crucial for identifying and mitigating potential hazards.
17-Inadequate resources allocated to health and safety: The DM mentioned that they
were constantly replacing workers, indicating potential issues with resource allocation.

Question 5 (b)
The injured worker's behaviours were probably influenced by the following factors
1-Based on Competence Job description of all Workers not properly defined as due to
understaffing on the day of the accident, the apprentice had assigned additional duties,
hence increasing his workload
2-The apprentice had assigned responsibilities to work lonely without sufficient support.
3-Without experience, the apprentice may not know what to do with children and may not
receive adequate training on how to manage their behavior.
4-Poor situational management resulted from a lack of documented procedure.
5-A failure in the normal monitoring of nursery facilities is suggested by the loose and worn
climbing frame rungs and the lack of frequent equipment checks.
5-The apprentice may not have had enough time to learn nursery safety and emergency
response procedures, causing confusion and inadequate handling of the situation

Answer sheet IG1-0033-ENG-OBE-V1 Dec23 © NEBOSH 2023 page 5 of 8

6-Due to a lack of sufficient staff, the apprentice was left in charge of a group of children
without adequate supervision, which unfortunately increased the likelihood of them being
unaware of safety rules.
7-The refusal to address safety concerns related to the climbing frame, combined with
visible wear and damage, created an unsafe environment, influencing the apprentice's
behaviour.
8-The apprentice may have been overwhelmed by the complexity of the task, which
involved supervising a group of children and managing outdoor activities. This could have
led to oversight of safety procedures.
9-The apprentice being sent home without receiving proper first aid and medical attention
suggests a possible issue with promptly reporting and addressing injuries.
10-The accumulation of multiple near misses, without any noticeable modifications to
procedures or equipment, indicates a lack of attention towards learning from and
mitigating potential hazards

Task 6: Determining uninsured costs incurred from the accident

near-miss record-keeping systems.
11-The expenses associated with giving employees further instruction or refresher
courses on health and safety procedures for the new workers.
12-The price of carrying out an internal inquiry into the mishap and this process also
takes time which is going to cost more to BB and this cost is also uninsured.

Task 7: Determining the benefits of inspections

Question 7
Prior to the disaster, the nursery may have benefited from health and safety inspection in
the following ways:
1-Legal Obligation Fulfilment-By ensuring conformity to legal responsibilities, inspections
would have protected the nursery from potential legal implications and financial penalties.

consistent equipment maintenance and repair, avoiding the use of dangerous apparatus
such as the faulty climbing frame to lower the likelihood of accidents.
10-Documentation and Record-Keeping Assurance: Tough audits ensure availability of
health and safety paperwork, reducing misunderstandings and noncompliance, illustrated
by an outdated regulation referencing the wrong institution.
11-Heightened Communication and Awareness: Inspections would have fostered a greater
comprehension of occupational safety and health requirements among workers, ensuring
that all were aware of legislation controlling excursions and equipment use.
12-Continuous Advancements: Inspections would have acted as a catalyst for constant
enhancement in facilitating health safety, identifying and correcting the areas of concern
for cultivating a safer workplace for workers as well as kids.
13-Proactive Accident Prevention: Inspections would have significantly decreased the
likelihood of injuries and the regrettable incidence concerning the trainee, by recognizing
and fixing possible threats and dangers in advance.
