A. ROLES AND RESPONSIBITIES


HSE Organization and Responsibilities is being established to define the roles, responsibilities
and duties of all employees (from top management to the lowest rank of worker) to efficiently
conduct the HSE management.

1. Organization Chart
CONTRACTOR shall establish Project safety organization and appoint registered safety manager
to comply with relevant legislation and to implement Occupational Health and Safety
Management System on site.

 Reporting Lines & Accountability


Clear lines of reporting will be established and communicated within the organization, to
ensure that the appropriate response to any incidents is properly managed and implemented
promptly. Each individual will be made to understand their responsibilities and
accountabilities.

The actual HSE organization of the Project is shown below chart.

2. Site HSE Responsibilities


CONTRACTOR recognizes the important role that all employees can contribute to the totalefforts
in preventing accidents at work and assist in achieving high standards of environmental control.
Their efforts are anintegral part of the company’s construction activities.

 Project Coordinator
The Project coordinator is the ultimate responsible for the implementation of the HSE MS, its
performance, and continual improvement.
In particular, the Project coordinator is responsible for the following :
 Ensuring that CONTRACTOR’s Top Management radiate their HSE commitment down
to all levels and functions of the Project organization;
 Allocating sufficient resources for the management and implementation of the
integrated HSE Management System;
 Ensuring that line personnel take ownership of the HSE Management System as
applied to the Project;
 Ensure, incollaboration with Section Managers, relevant Project HSE requirements are
conveyed to Sub-Contractors and implemented;
 Maintain HSE leadership for Site activities.

 Site Construction Manager


The Site Construction Manager shall ensure that the Site is maintained in a safe condition and
that construction activities are performed in accordance with applicable HSE procedures by
carrying out regular inspections of the work with supervisory personnel and participating in
formal HSE inspections/audits. In more detail, the Site Manager shall :
 Provide instructions to his personnel for the correct implementation of the HSE
requirements;
 Demonstrate and maintain commitment to relevant Project HSE Policy and objectives;
 Be responsible for ensuring that all Site activities are executed ina safe,
environmentally responsible, and controlled manner;
 Attend relevant HSE Meetings;
 Ensure that all Site construction personnel are skilled, qualified and adequately trained
for their job assignments;
 Ensure, incollaboration with Project C, relevant Project HSE requirements are conveyed
to Sub- Contractors and implemented.

 Site HSE Manager
The Site HSE Manager (SHSEM) shall lead the HSE organization on Site. The SHSEM shall
monitorregularly and systematically the actual implementation of the HSE Management
System on Site, verifying that the Project HSE Policy is enforced and the HSE requirements are
fulfilled. The SHSEM shall convey monitoring results to the Project Coordinator. In addition,
he shall:
 VEnsure updating and implementation of the Project HSE Plan as well as other relevant
Site HSE documentation;
 Promote continuous review and improvement of the HSE MS implementation on Site;
 Review and comment on HSE and related construction document according to Site
situation;
 Verify that Sub-Contractors’ HSE Plans are in line with this document and approve
them accordingly;
 Monitor the planned execution of work, evaluating the changing circumstances of the
Site in order to identify potential hazards and communicate them to line management;
 Contribute to identifying and promoting safe work practices and procedures;
 Ensure that all personnel, vehicles, and equipment meet legal and contractual HSE
requirements;
 Produce Site HSE records;
 Notify the PM and CM of any HSE related incident/accident involving personnel,
equipment, property or environment;
 Review and analyze incident reports, medical logs, and other relevant information to
identify trends and support in defining related preventive and corrective actions;
 Liaise with CONTRACTOR construction management for any HSE issue;
 Liaise with Sub-Contractors' or Vendors’ HSE personnel;
 Arrange for internal HSE audits;
 Maintaining effective communication between COMPANY and CONTRACTOR on HSE
matters.

 HSE Inspector/Officers
The HSE Inspectors/Officers shall:

 Monitor the implementation of the HSE Management System in their assigned area
and support the relevant construction Supervisors on all HSE matters;
 Verify that relevant HSE information and training is provided to all personnel working
in the assigned area;
 Collaborate with Supervisors in assessing training needs for personnel in the assigned
area;
 Attend all relevant meetings;

 Reports HSE findings to the relevant construction personnel as well as the SHSEM, and
follow-up the status of implementation of the identified preventive and corrective
actions;
 Carry out any other assigned HSE activity.

 Security
a. The security leader at each shift shall issue to all site personnel numbered identification
badges that each employee must wear upon their clothing.
b. The security supervisor must maintain a close control of all the badges issued.
c. The control records must include ;
 The Badge Number.
 The name of the person whom the badge is issued to.
 The date issued.
 Valid period.
 HSE Induction Record
d. HR Manager shall submit and maintain a badge status list changes to Security Team
whenever a new employee begins work or an employee is terminated.
e. The badge system will be in line with the HSE Induction Training Program. Personnel who
pass Induction Training will be issued with badges as refer to HSE Training Procedure
f. There will be a deposit system to issue the badge. For one badge issuance, it has a value
of IDR 30,000 (thirty thousand rupiahs). The total value of deposit is referred to the total
badge issuance to the Contractor/Vendor/ Client/Siemens. Deposit should hand over
directly to HR & Admin Manager PT. Dewata Makmur Bersama by
Contractor/Vendor/Client/ Administrative Representative and the receipt will be given
accordingly.

 Sub-Contractors/Vendors
 Every Sub-Contractor/ Vend or working on the Project shall :
 Comply with all Project HSE requirements;
 Strictly adhere to this HSE Plan;
 Prepare their own HSE Plan in line with the present document and submit it to
CONTRACTOR for approval;
 Ensure that their own organization at Site include an adequate number of HSE staff;
 Ensure that their personnel on Site are aware of HSE requirements;
 Identify and evaluate all job hazards for their workforce, apply all necessary preventive
and protective measures and issue protective equipment to them as necessary for the
work to be performed;
 Cooperate with CONTRACTOR in investigating incidents to identify and enforce
corrective actions to prevent recurrence;

 Attend relevant HSE meetings and implement any identified action item;
 Immediately notify CONTRACTOR of any incident.

 All Project Personnel/Employees


All personnel employed on the Project are responsible and accountable for:

 Accepting individual responsibility for their own safe behavior as well as sharing
responsibility with their fellow workers and line supervision to “look out for their own
well as their co-workers” safety;
 Complying with and constructively participating in the Project HSE activities of the
Project Management System;
 Work in a safe manner at all times and contribute to the implementation of the “
Incident and Injury Free” philosophy;
 Comply with the safe working policies, procedures and practices adopted by the
COMPANY as part of the safe working system;
 Co -operate with their COMPANY in ensuring that their workplace is as safe and
healthy as is reasonably practical;
 Report to their supervisor any hazard or potential hazard in the workplace or any
incident, personal injury, accident or near miss that may have occurred during the
course of the work to the supervisory staff immediately;
 Observe all warning signs and notices;
 Attend toolbox meetings and pre start meetings;
 Wear PPE appropriate for the workactivities.

3. Management of Change for Organization & Personnel


The Management of Change is geared to safe process and equipment modification. No
modification shall be made to a plant, equipment, control systems, process, conditions and
operating procedures without authorization from a responsible manager or his representative. All
modifications must be evaluated for HSE impact and must be properly documented before being
implemented. This ensures such change had been carried out in accordance with management
approval. CONTRACTOR recognizes that significant hazards can be created due to changes in the
following aspects of the Project:
7.3.1 Changes in scope of work;
7.3.2 Changes in method of work;
7.3.3 Changes in climatic and weather conditions;
7.3.4 Changes to management systems;
7.3.5 Changes to communication systems;
7.3.6 Equipment changes; and
7.3.7 Personnel and contractor changes.

4. Violation and Infringemen
It is the Company's Policy that all personnel shall work safely at all times. Any employee who
violates the Company Policy, acts in unsafe manner or who fails to apply to a proper safety
standard will be subject to a disciplinary action described in the Company Policy. Contractors and
other personnel who fail to comply with the Company rules & regulations will be subject to a
disciplinary action which may lead to rejection of services and suspension of future work with
Contractor.

5. Working within company’s premises


Working within Company premises requires everyone to use a proper entry permit badge or ID
badge. Detail procedure to obtain a badge is described under Externa l & Security Rules. Anyone
who enters or visits a working area for the first time, or has not entered the area in the last 6
months, shall be given a Safety Induction. Authorized HSE personnel or Line Manager shall ensure
that the appropriate safety induction is conducted.
Working within the project or Plant Facilities must also comply with the following requirements:
 Use proper PPE
 Cellular phones must be switched off.
 Any radio communication is not allowed unless approved by authorized personnel.
 No ignition sources including matches or cigarette lighters are allowed.
 No picture taking or use of flash light unless approved by authorized personnel.
 No work can be done without approval from authorized personnel.
 Read and follow any warning sign boards.
Anyone found working without receiving the appropriate safetyinduction or failing to comply with
the above rules will be subject to disciplinary actions both to the violators and his/he r superior.

6. Employee’s Requirements
All employees must be in good physical condition, i.e. appear healthy, have adequate hearing and
sight, possess all limbs, do not suffer from vertigo, etc. that are proven by medical checkup record.
The Contractor shall ensure that during construction period, every worker shall be covered and
protected by labor insurance such as Jaminan Sosial Tenaga Kerja.

7. Emergency Preparedness & Evacuation


Please refer to CONTRACTOR’s Emergency Preparedness & Response Plan.
