Incident Type: EVOLIS Twin Plus Equipment Breakdown

Date of Incident: September 16, 2023

Scope of Incident:

1. The plate was inserted in the plate carrier of the machine, ensuring proper placement.
2. Upon processing the start button, The carrier went in the dedicated slot and commenced
the processing.
3. The process was seamless during the initial insert. However in the second attempt, the
system prompted ‘’Plate carrier not detected’’.
4. Processing was then aborted.
5. After terminating the process, the metal plate carrier was detached, to note, the plate was
still intact in the carrier.
6. Correspondence with Mr. Carlo Magnampo, Lifeline diagnostics application specialist, was
initiated to inquire troubleshooting procedures.
7. Instructions provided were:
a) Remove the metal plate carrier
b) Re-insert back to the movable tray
c) Re-initialize the machine
8. After re-initializing the machine, new work list was loaded.
9. Standards and reagents were assigned.
10. Plate rack was placed on the metal plate carrier.
11. Error message ‘’Plate carrier not detected’’ alarmed.
12. Work list was aborted.
