1.4 Task overview

The purpose of this task is the livening of the Module U Clean Utility systems 818-U080 DH3 Generation, 818-U081 DH3 Distribution and 818-U091 HVAC
Steam (DH3) Distribution.

The livening activities will be completed in several controlled phases across all applicable systems:
1. PHASE 1 : Livening of all Utilities in 818-U080 DH3 Generation Skid
2. PHASE 2 : Steam Generator Start-Up and Livening of the Steam Distribution (818-U081 DH3 Distribution)
3. Phase 3: Livening of Steam Distribution 818-U091 HVAC Steam (DH3) Distribution

1.5 Requirements

2. All operatives involved to have appropriate task-specific PPE.
3. Complete Scope of Systems involved in livening walked down under multidisciplinary teams completed and punch list produced.
4. System is already mechanically completed and category A punch list items are closed.
5. Evidence of Pressure testing of 818-U080, 818-U081, 818-U091 and 818-U115 Pressure test and Blowdown completion and compliance with full
certification available.
6. LOTOTO Plan for Living correctly Implemented across all applicable systems listed in section 1.4.
7. Lonza EMR are available to support remotely with any DeltaV issues during the start-up of the skid

1.7 Methodology

PHASE 1 : Livening of all Utilities in 818-U080 DH3 Generation Skid

This Phase is the introduction of the utilities into the system boundary of steam generator 818-U080 DH3 Generator.

1. As a baseline activity before any livening activities. Verify that Correct signage, Barriers and insulation is in place.
2. Verify that the following Valves are locked closed 818-U101VH105, 818-U081VH103, 818-U081VH104, 818-U081VH106, 818-U233VH266, 818-
U233VH267, 818-U116VH113, 818-U115VH126, 818-U091VH108, 818-U091VH109, 818-U091VH110, 818-U091VH111, 818-U091VH106
3. Verify the valve positions of the are correct as listed below
a. Open Valves: 818-U080VH103, 818-U080VH104, 818-U080VH101, 818-U080VH102, 818-U080VH106, 818-U080VH109, 818-U115VH126,
818-U116VH112, 818-U081VH101, 818-U081VH105, 818-U081VH107, 818-U081VH109, 818-U091VH112, 818-U091VH115, 818-U091VH116,
818-U091VH117, , 818-U091VH102, 818-U091VH103

4. Liven MKWP supply and return lines into 818-U080 skid. Unlock MKWP Supply and Return valves 818-U233VH266 and 818-U233VH267. Crack open
818-U233VH266 valve, Verify that water pressure builds up on skid on Pressure indicator 818-U080PT2021. When pressure has stabilized Open 818-
U233VH266 fully and then slowly open 818-U233VH267 to complete supply and return loop. Verify there are no leaks

5. Liven Condensate Return from 818-U080 Skid. Remove lock and open Valve 818-U116VH113. Verify there are no leaks
6. Liven WBU Generation Supply to 818-U080 Skid. Verify that the Automatic Valve U080YV4098 is not isolated. This valve is automatic and will be used to
introduce water into the system when producing steam.
7. Liven Instrument Air to 818-U080 Skid. Verify that Valve 818-U020VH121 is unlocked and air can be introduced to skid.
8. Verify that Drain connection to 818-U080 is good and that there are no spools in place online to Wan tank
9. Liven D8 Steam to 818-U080 Skid. Unlock Valve 818-U101VH105. Slowly crack 818-U101VH105 open and allow line to fill with steam upto Valve 818-
U080YV2130. Verify there are no leaks.

PHASE 2 : Steam Generator Start-Up and Livening of the Steam Distribution (818-U081 DH3

This phase is the start-up of the steam generator and subsequent verification that the steam produced will livening safely and properly in the downstream
steam distribution header

1. Verify that steps listed in Phase 1 are completed


2. Verify that all signage and Warning tape is in place in areas when livening is taking place.
3. Verify that Lonza EMR are available to support online for any automation issues.
4. Verify the following LOTOTO is in Place:
a. LOCK Removed and Valve Opened : 818-U115VH126
b. Valve Locked Closed: 818-U091VH108, 818-U091VH109, 818-U091VH110, 818-U091VH111, 818-U091VH106, 818-U081VH103, 818-
U081VH104, 818-U081VH106
5. Verify the valve positions of the are correct as listed below
a. Open Valves: 818-U080VH103, 818-U080VH104, 818-U080VH101, 818-U080VH102, 818-U080VH106, 818-U080VH109, 818-U115VH126,
818-U116VH112, 818-U116VH113, 818-U081VH101, 818-U081VH102, 818-U081VH105, 818-U081VH107, 818-U081VH109, 818-U091VH112,
818-U091VH115, 818-U091VH117, 818-U091VH102, 818-U091VH103
b. Closed Valves : 818-U081VH103, 818-U081VH104, 818-U081VH106, 818-U081VH110, 818-U081VH108 (Closed for now, Valve will opened at
a later stage), 818-U091VH114, 818-U091VH113, 818-U091VH116 (Closed for now, Valve will opened at a later stage), 818-U091VH104, 818-

6. Only once step 2 & 3 are confirmed can we proceed. With Support with the Vendor (BWT will be onsite with Specialist commissioning engineer to
start-up system) start up Generator using DeltaV Logic . The Logic will draw in water from the upstream WBU Generation system (818-U174 system)
and heat the water with D8 Steam (818-U101). Be very vigilant for of the system and check continuously for leaks with the 818-U080 Skid.

7. DH3 Steam will be produced in the generator column (818-U080K10) and will be directed into the 818-U081 DH3 Distribution System. Steam will travel
as far 818-U081VH108 (Closed but not locked). Allow for Header to pressurize. The Setpoint of the Steam pressure produced by the DH3 Generator is
3bar. Verify the pressure on DeltaV on U080PC0020 and in the field on pressure gauge 818-U081PI101. Allow for pressure to stabilize before
proceeding . Note Condensate valve 818-U091VH112 may need to be pulsed open and closed to clear condensate. Verify that there are no leaks.

PHASE 3 : Livening of the Steam Distribution (818-U091 HVAC Steam (DH3) Distribution)

1. Only Proceed when the 818-U081 Header has stabilized at 3bar and all steps with Phase 2 have been completed. Have a member of team located at
level 2 where the riser is located with radio. They will verify that between level 1, 2 & 3 that there are no leaks on the riser when steam is introduced.
2. Verify the valve positions of the are correct as listed below
a. Open Valves: 818-U080VH103, 818-U080VH104, 818-U080VH101, 818-U080VH102, 818-U080VH106, 818-U080VH109, 818-U115VH126,
818-U116VH112, 818-U116VH113, 818-U081VH101, 818-U081VH102, 818-U081VH105, 818-U081VH107, 818-U081VH109, 818-U091VH112,
818-U091VH115, 818-U091VH117, 818-U091VH118, 818-U091VH116, 818-U091VH102, 818-U091VH103
b. Closed Valves : 818-U081VH103, 818-U081VH104, 818-U081VH106, 818-U081VH110, 818-U081VH108 (Valve will opened in next step of
Phase 3), 818-U091VH114, 818-U091VH113, 818-U091VH104, , 818-U091VH119, 818-U091VH101, 818-U091VH111, 818-U091VH106, 818-
U091VH110, 818-U091VH109, 818-U091VH105
3. Crack open 818-U081VH108 a little. This will allow steam to travel into 818-U091 HVAC Steam (DH3) Distribution. We should see a an immediate drop
in pressure on U080PC0020 and 818-U081PI101. Allow for the 818-U091 system to pressurize with steam. The system will pressurize until
U080PC0020 and 818-U081PI101 reach setpoint pressure of 3bar. During this time ensure there are no steam or condensate on 818-U091 Riser on
steam or condensate lines.
4. Once pressure has been achieved system can be considered in a live state. Note 818-U091VH116 and 818-U091VH103 may need to be pulsed opened
and closed to clear condensate.
5. Verify that condensate is returning to Condensate tank 818-U115B1

Safety relevant steps:


1. Activity must be of normal working hours and the Level 0 utility Area should be free of all personnel not directly involved with activity. Area will be
demarcated with Signage and warning tape for the activities
2. Personnel are wearing the required PPE.
3. Radios will be issued prior to works commencing, and a radio check carried out to ensure communication is confirmed between all parties prior to
works commencing.
4. LOTOTO has been completed with valves locked closed and relevant blinds tagged
5. All Safety signage is in place
6. Level 0 Clean utility Area around steam generator and riser will be cordoned off
7. Construction personnel are informed of commissioning activities via daily whiteboard
Relevant permits are in place and signed off for livening and commissioning activities to be performed

2.0 Risk Assessment


2.1 Instructions
1. Break the job down into steps - Be as specific as possible. Write each job step in a new row of column A (Activity). It’s a good idea to leave an extra row
between steps in case you need them later. Add additional rows to the table as needed,
2. Determine the hazards associated with each job step and write them in column B (Hazards). Use the examples of hazards provided below to trigger your
thinking about different types of hazards and energy sources. In the next column state who could be harmed and how that may occur (C).
3. Considering the hazards involved without any control measures being in place, determine the worst credible severity of an incident that could result from
this job step. Use the severity table provided. Write the severity number in column D (Severity).

9. Consider the risk rating against the thresholds provided in the Overall Risk Ratings.
9.1. Very low and low risk ratings of 1-4. This level is generally considered acceptable provided that all control measures are applied.
9.2. Medium risk ratings of 5 -12. Additional control measures should be considered to determine whether it is practical to further reduce the risk further.
If additional control measures may be required, write them in the Control Measures column on the same line or a new line below. Rescore the
Severity, Likelihood, and Risk taking into consideration this additional corrective action.
9.3. High and Very High-risk ratings of 15-25. Additional control measures must be considered to determine whether it is practical to further reduce the
risk. If additional control measures will be implemented, write them in the Control Measures column on the same line or a new line below. Rescore

2.2 Risk Measurement

The main tool used for assessing risk in this system is a 5 x 5 matrix. The sides are labelled Severity x Likelihood as shown below.

If the multiple of Likelihood x Severity is in the range of 1-4 the risk is considered low and provided the correct processes are followed no further control
measures are considered necessary.

Medium (M) Risk (Orange)


If the multiple of Likelihood x Severity is in the range of 5-14 the risk is considered medium and further control measures will be in place to reduce the risk.
After the control measures are put in place, the risk will be re-assessed in order to determine the impact of the risk with the control measures in place. A
decision will be taken as to whether the risk is now acceptable before work is allowed to proceed.
