Colour Vision Test is only applicable to students who have been offered the following courses.

I certify that the above-named student has undergone the Colour Vision (Ishihara) Test and the result is as
follows:

Partial Colour Deficiency
Complete Colour Deficiency
• The information provided will be kept strictly confidential and used for assessing fitness for course
admission only. Fees incurred for the test are to be borne by student.
• Complete Part A of this form and bring along with you to a Singapore registered medical
practitioner for the test. You may visit a private clinic, your own family doctor, or any of the specified
medical centres listed in the next page.
• Please ensure your medical report is completed before you leave the medical centre, i.e. the doctor
has certified whether you are fit to pursue the selected course of study at NYP. You will be deemed
unfit unless certified fit.
• Please wear your glasses or contact lens for the colour vision test.
• For International Students, the additional Medical Examination Report is required by ICA for your
Student’s Pass Application. The report can be downloaded from NYP Online Enrolment System or NYP
Enrolment Website : www.nyp.edu.sg/enrolment.
Important :
• If you are unable to complete the test before the deadline given, please inform us via email at
NYP_Admissions_Office@nyp.edu.sg. Failure to submit your medical report may result in you
being deemed medically unfit to pursue the course.
• If you have colour appreciation deficiency, you may encounter difficulties meeting the course
requirements and expectations. Please inform us immediately via email at
NYP_Admissions_Office@nyp.edu.sg with the email subject “Colour Vision Deficiency”,
stating clearly your Student Admission Number, Name, Contact Number and Course Offered.
• Students with severe colour appreciation deficiency will be assessed on a case-by-case basis to
determine whether they can continue with the offered course or be recommended for a course
transfer.
