PARENT’S WAIVER

I, ___________________, give our full consent and agree to join our child,
________________, in the community service project named Fun Caravan on the 3rd
of March 2024, from 8:00 AM to 12:00 NN, the event will take place at 40 Paraiso,
Lower Bicutan, Taguig.

We acknowledge that this initiative is a necessary academic requirement and

therefore grant our approval for his/her involvement. We understand that this project
aims to bring joy and help to many children in need within our community.

We acknowledge that this community service project will involve various activities,
such as organizing events, providing support to children, and contributing to the overall
success of the Fun Caravan. As parents, we fully support our child’s initiative to
engage in this meaningful project and believe that our involvement will also benefit the
community as a whole.

By signing this consent form, we confirm that we understand the nature of the
project and the responsibilities that come with our involvement. We also agree to bear
any responsibility for any unforeseen circumstances that may arise during the course
of this activity.
